On 03/16/2023 at 8:40a.m.
.LPA met with the Director Kirstin Mc Duffie, who guided LPA on a tour of the facility. There were fifteen (15)children in care and four (4) staff upon arrival.During the investigation LPA interviewed, the Director, Staff, Childrenand obtained a copy of the Sign in Sheet for 01/09/223.Maintenance Request Form, Enviro- Master, PersonnelReport, Daily Classroom Checklist, Facility Roster, Orkin Bill, Academy for Early Learning Parent Handbook, andsupporting documentation. In addition, LPA interviewed staff, children, and the Director.The Department conducted a full investigation, which included staff interviews, and interviews with relevant parties,as well as a record review, including documentation as related to the allegationLPA did not observe, nor was information provided via interviews that the staff do not take measures to keep center free of inspects or spiders.Therefore, the allegation of Physical Plant is deemed unsubstantiated. Meaning, althiugh the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur.
