On 01/16/2025 at 04:41 p.m., officer of the day Licensed Program Analyst (LPA) Cristina Castellanos received a call from Aolelani Lutu, Site Supervisor in connection to an Unusual Incident that occurred at the facility in regard to a child being left unattended from 7:54am to 9:15am. Through documentation, interviews, and a physical plant inspection of the site of the incident, it was revealed that at the time of the incident while the children were transitioning from the PreK1 classroom (the receiving room) to PreK3 (breakfast room) staff members S8 and S1 left a child (C1) unattended in the PreK1 classroom. At approximately 9:15am the classroom’s staff member (S10) found C1 sleeping in the classroom alone. C1 was left unattended for approximately 1 hour and 21 minutes.
Per the Site Supervisor Lutu, C1’s parents were notified on Wednesday January 22, 2025 due to parents requiring a Spanish translator.
Based on observation, record review and interviews of all relevant parties, it was found that the facility has been operating out of ratio and staff left a child unattended, the preponderance of evidence standard has been met, therefore the above allegations are found to be SUBSTANTIATED.
There were two deficiencies cited during today’s visit in accordance with the California Code of Regulations, Title 22, Division 12, and Chapter 1. See LIC 9099-D for additional information. As well as a Civil Penalty Assessment - Immediate.
