An unannounced Required – 1 Year Inspection was conducted on 12/16/2022 at 11:30 AM by Licensing Program Analyst (LPA) Katrina Chicote. Facility is currently licensed for a capacity of 51 but currently have 25 enrolled. LPA announced purpose of visit and initially met with Melissa Allen, Head Teacher, who granted entry to the facility. Early Learning Center Manager (ELCM), Shannon Houston, arrived at facility at 11:50 AM and joined inspection. The program currently operates Monday thru Friday from 7:45 AM to 2:45 PM. LPA was taken on a guided tour of the facility of both indoors and outdoors.

At the initial start of the site visit, there is a total census of 15 children and a total census of four teachers present. The facility is made up of two classrooms RM 1-Preschool and RM2-Early Head Start (EHS). LPA observed nine children in RM 1 with two staff (S1 and S2) and six children in RM 2 with two staff (S3 and S4).

Beginning at 11:34 AM, furniture and equipment were inspected for age appropriateness and good repair. Telephone service, heating, lighting and ventilation were evaluated in each classroom. There were cubbies observed as storage for children's belongings in each classroom. LPA arrived during lunch time and observed children eating lunch. LPA observed trash can available with tight fitting lid. LPA observed blue cots set up for nap time each with fitted sheet and blanket provided. Per Head Teacher, sheets are provided by facility and washed by laundry service while blankets from home are taken home weekly. LPA observed nap equipment is set up to not hinder walkways and or block access to exits at time of inspection. LPA observed learning stations in each classroom for dramatic play, art, manipulatives, and other age appropriate toys free of loose and sharp parts available in each classroom. LPA observed a quiet play area available for children with soft seating and books well maintained. LPA observed floors and carpets to be free of hazards at time of inspection. Availability of drinking water was reviewed. LPA observed water coolers with cups provided in each classroom. Age appropriate sinks and toilets were inspected for availability, good repair,

water temperature, toilet paper, area safety and sanitation. LPA observed RM 1 to have three bathrooms
totaling seven toilets and eight sinks available for children and RM 2 to have one toilet and one sink and diaper changing table available that meets Title 22 Regulation requirements at time of inspection.

Beginning at 11:45 AM, outdoor equipment was inspected for safety, cushioning material, good repair and age-appropriateness. Required shade and fencing were inspected. LPA observed play structure with steps and slide with cushioning material below to absorb fall all well maintained. LPA observed concrete through out outdoor play area with balancing poles, plastic play houses, and plastic play kitchens all well maintained, benches with umbrellas and other shade available for children were observed. LPA advised staff that the children need to be within the direct care and supervision, including visual supervision of the teacher(s) at all times. The children have access to drinking fountains during outside play as well as water coolers are brought out. Play area was inspected for hazards and inaccessibility to bodies of water; no bodies of water or hazards observed.

Snack/lunch menus were reviewed and observed posted. Per Head Teacher, all meals and snacks are delivered from Head Start kitchen. At 11:47 AM, LPA observed food preparation areas for safety, cleanliness and proper equipment. Facility serves breakfast, lunch, and snack.

Teacher-child ratios were observed and staff names recorded. All staff and employees have obtained a criminal record clearance or criminal record exemption as a condition of employment with the Long Beach Unified School District. Care and supervision was evaluated to determine if the basic needs of children are met and appropriate. Sign-in and out sheets and procedures were reviewed. LPA observed all children present were signed in at time of inspection. Personal Rights of children were observed by LPA.

Beginning at 12:20 PM, tsaff and Children’s Records were reviewed. Children's records were reviewed for completeness and recorded on LIC 857. LPA also reviewed staff records. The review of Staff records was documented on the LIC 859. Staff present have proof of the AB 1207 Mandated Reporter Training certificate on file. All staff have been given on the-job training sanitation principles, housekeeping, including universal health precautions. There is at least one staff member present trained on Pediatric First Aid & CPR. CPR card expires on 03/2023 for S1. LPA did not review confidential forms and information due to the files being stored at the administrative office.

A first aid kit is kept in hallway and in teacher cabinet in each classroom which LPA inventoried for supplies. At 11:37 AM, carbon monoxide detectors observed operable and smoke detectors are present in the facility. Fire extinguishers have been serviced on 09/01/2022. Disaster drill log was available during visit, last drill conducted on 12/15/2022. Hazardous items including cleaning compounds were stored inaccessible to children. LPA observed these items stored on high shelves during inspection. Per Early Learning Center Manger, sick/isolation area for children is in small alcove in RM 1. LPA observed medicines stored for three children (C1, C2, and C3) stored in RM 1 per Title 22 Regulation requirements.

This facility provides Incidental Medical Services – IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. The facility was found in compliance per Title 22 regulations, there will be no deficiencies cited today, 12/16/2022.

The Notice of Site Visit (LIC 9213) – must remain posted for 30 days during the hours of operation after each site inspection by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00.
