On 03/10/2025, Licensing Program Analyst (LPA) Lisa Clayton conducted an unannounced Required 3-year Inspection on the school age license for STAR – Canyon. LPA met with Site Director Carlos Velado. LPA observed 10 children in the TK - 1st grade group and 5 children in the 2nd - 5th grade group being supervised and cared for appropriately by 4 fingerprint cleared staff.

Children in are signed in on Monday, Wednesday and Friday at 2:55 pm and Tuesday and Thursday they are signed in at 1:45pm.

LPA Clayton toured the classroom, restrooms, cafeteria, and play yards for a Health and Safety inspection.

The Program operates on the Canyon Elementary School campus. The program currently operates in the following areas: room #16, and the yard. Site Director stated the children do homework, science, arts & crafts in the classrooms, and sports, dance and recreation on the yard. Furniture and equipment are in good condition, free of sharp, loose or pointed parts. All materials and surfaces accessible to children are toxic free. The isolation area is located in the nurses office in the main building. LPA inspected the outside play area and surface of the outdoor activity space is maintained in a safe condition and is free of hazards.

LPA Clayton observed the area behind the Star Classroom (previously part of the play yard) to be gated off and under construction. Per Site Director Carlos the school is constructing a 2-story building to replace the bungalows. LPA Clayton reminded Site Director that prior to construction or alterations, the licensee shall notify the Department of the proposed change(s). LPA Clayton instructed Site Director Carlos to contact the Department and schedule an inspection of the new building prior to use.

The facility provides snacks to children in care which is provided by the Star Program. LPA Clayton observed and inspected the food storage areas designated for the after-school program. LPA Clayton reminded Director Carlos to put the date on all food once opened and add the expiration date to any food items removed from the original container. Menus are posted monthly.

There is no swimming pool or other bodies of water on the premises. There are no firearms or ammunition allowed or stored on the premises. Disinfectants, cleaning solutions, medication and other hazardous items are made inaccessible. LPA observed multiple first aid kits. LPA Clayton reminded Director and Assistant Director to check the kits for expired products regularly.

The children have access to 1 boy’s bathroom & 1 girls bathroom inside of the main building, and a bathroom bungalow with separate boys and girls bathrooms. All toilets and handwashing facilities are in safe and sanitary operating condition.

Drinking water is available inside and outside.



Facility was observed to have a centralized smoke detector/alarm system. LPA Clayton observed a fully charged 3A40BC Fire Extinguisher in the classroom. Site Director Carlos stated that the Fire Department tested the alarm systems on Monday February 24, 2025. LPA Clayton instructed Site Director to request the Fire Department Inspection report for their records.

LPA Clayton reviewed 5 children’s files and observed files to have current contact information of parents and authorized representatives, and authorization for medical treatment, signed Parent’s Rights and signed Personal Rights receipts.

LPA Clayton reviewed 5 employee files and observed licensing forms and immunization records required for childcare providers.

Site Director was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Childcare Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.

Incidental Medical Services (IMS) policy was discussed, and LPA Clayton provided Site Director with an IMS Plan sample for his reference. For IMS information see PIN 22-02-CCP. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. LPA Clayton informed Site Director of the MyChildCarePlan.org website; a consumer education website that helps families obtain childcare by connecting them to childcare providers and Resource and Referral Agencies (R&Rs) throughout California.

Per Title 22, Division 12, Chapter 3, of the California Code of Regulations and/or Health and Safety Codes, no Deficiencies were cited.


During the exit interview the report was reviewed and a copy was provided to Site Director Carlos.
LPA Clayton posted a Notice of Site Visit which must remain posted for 30 days.
