On 03/06/2024 at 9:00 AM, Licensing Program Analysts (LPAs) Katrina Chicote and Claudia Kam conducted an Unannounced Case Management Inspection and met with Facility Representative (FR) Maria Rodriguez, Director. LPAs disclosed the purpose of the inspection and was granted entry into the facility and was provided guided tour. Facility is made up of 12 classrooms but currently only using eight classrooms at this time. Census was taken.

The purpose of today's inspection was to discuss the water lead test results. Results received by RO indicated additional water sources not discussed during visit on 03/01/2023, labeled W, to have action level exceedance (ALE) of lead with results of 22.6 ppb and water source labeled VR, to have ALE of lead with results of 6.2 ppb. Per FR, these water sources are in area of facility that is currently not being used but also have been tapped off and completely removed. LPAs observed water source labeled W to be in Room 11, to be capped off and in a room not being used. LPAs observed Room 11 to have furnitures stored and stacked against walls and no children or staff in classroom. LPAs observed water source labeled VR to be drinking fountains in outdoor play area to be completely capped off and in area that is not in use by children at time of inspection with fences surrounding the area making it inaccessible at time of inspection.

LPAs observed and took pictures of various water sources and confirmed water sources to be permanently removed at time of inspection. LPAs observed water pitchers with paper cups available in facility for additional drinking water source though there are drinking fountains available in classrooms and in Outdoor Play Area that have been remediated and no longer have ALE results.

A notice of site visit was given and must remain posted for 30 days.

Exit interview was conducted and report was reviewed with Facility Representative.
