At 10:45 AM Licensing Program Analysts (LPAs) Roxana Lopez and Saul Valenzuela conducted an unannounced required inspection. A risk assessment was conducted prior to entering the facility. LPAs met with Principal Sal Sandoval, who guided the LPAs on a tour of the facility. This is a preschool age program licensed for 178 children which operates Monday – Friday 7:00 am to 5:30 pm. Per Principal there are 100 children enrolled.

All areas identified on the Facility Sketch were inspected. The following staff was present during this inspection: Room # 1: Staff #1, Staff #2, Staff # 3 & Staff # 4 with 15 children. Room # 2: Staff # 5, Staff # 6, Staff # 7 & Staff # 8 with 16 children. Room # 3: Staff # 9, Staff # 10 & Staff # 11 with 13 children. Room # 4A Staff # 12, Staff # 13, Staff # 14 & Staff # 15 with 14 children. Room # 4B Staff # 16, Staff # 17, Staff # 18 with 15 children. Room # 5: Staff # 19, Staff # 20, Staff # 21 & Staff # 22 with 17. Teacher-child ratios were observed to be in accordance with Title 22 Regulations. Staff names were recorded. The licensed facility is within the conditions, limitations, and capacity specified on the license.

At 11:18 am LPA’s observed 5 children unsupervised in the hallway/ cubby area. Principal walked into the children's restroom area to find the nearest teacher. Teacher in the restroom disclosed that the children were supposed to be supervised by another teacher. This poses an immediate risk for children in care.

LPAs observed the facility to be clean, sanitary and in good repair. Furniture and equipment was inspected for good repair, free of sharp, loose, or pointed parts. Classroom was inspected to ensure that the floors have a surface that is safe and clean. ----------------------------- pg.1 of 5 -----------

The nearest teacher was observed in the children's bathroom- per teacher there was supposed to be another teacher supervising the area, which poses an immediate health, safety or personal rights risk to persons in care.

LPA’s did observed one toilet to be out of order- Per Principal there is a work order on file- all washing facilities are in safe and sanitary operating conditions. All materials and surfaces accessible to children are toxic free. At this time, the staff lounge is used as an isolation area. Parents are contacted immediately when children are determined to be ill and staff are ensuring that children with obvious symptoms of illness are not being accepted.

The facility provides breakfast, lunch and snack. Menus were reviewed to ensure that they are being posted at least one week in advance and visible to an authorized representative. Per Principal meals are dropped of daily by the cafeteria located next door in the Castelar Elementary. All food preparation, and storage areas are clean, free of litter, rubbish, and rodents/vermin. All food is protected from contamination, and LPAs inspected that any contaminated food is discarded immediately.

There is drinking water available in the classroom via individual water bottles- which are taken outside for children to drink. All storage containers for solid waste, including moveable bins, have tight fitting covers on and are in good repair.

LPAs observed that there is smoke and carbon detectors in the facility and were found to be operable. The last Disaster and Fire Drill was conducted on 1/12/2024.

LPAs observed sleeping arrangements in form of mats- sheets are wash every week by a linen service.

At 11:35 am LPA’s observed a small trampoline and two accessible unlocked storage containers. The big storage container located on the right side of the yard contained ladders, dolly’s and various exterior paint cans accessible to children. The smaller containers located on the back of the playground contained various exterior paint cans, boxes of chemicals labeled keep out reach of children and shovels/tools. Pictures were taken- this poses an immediate risk for the children in care Principal locked the storage containers during the inspection.

Outdoor playground equipment is in safe condition, free of sharp, loose or pointed parts. The surface of the outdoor activity space is maintained in a safe condition and is free of hazards.------ pg. 2 of 5 --

All areas around or under high climbing equipment, swings, slides, and similar equipment are cushioned with material that absorbs a fall. There is adequate shade in the play yard.

Sign in and out sheets were reviewed to ensure that the person who signs the child in and out uses their full legal signature and records the time of the day.

There is at least one person trained in CPR and Pediatric First Aid present during this inspection. All staff have received an active criminal record clearance as a condition of their employment with the Los Angeles Unified School District.

LPAs did not review complete staff files during the inspection due to the staff files being stored at the administrative office. LPAs will review staff files at a later time and date. If staff files were to be incomplete during the time and date that LPA reviews files, a case management inspection will occur at the facility to reflect the incomplete files.

AB1207 Mandated Child Abuse Reporting – On or before March 30, 2018 any person who works in a child care facility shall complete the training and renew the training every 2 years. LPA reviewed storage of medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test. ---------------------------------------------------pg 3 of 5 ----------------------------------------------

For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1- CCP). LPA verified that the lead testing was completed in accordance to the Written Directives outlined in PIN 21-21.1-CCP.

Facility representative was informed of the MyChildCarePlan.org website; a


consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

LPA advised the licensee to access forms, regulations and quarterly updates on the Child Care Licensing website at: www.ccld.ca.gov

Facility representative was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

Based on the LPA’s observations and records reviews, the following deficiencies listed on the attached LIC 809D (deficiency page) are being cited in accordance with California Code od Regulations Title 22. Deficiencies that are being cited need to be cleared to protect the children’s health and safety.

A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
