Licensing Program Analyst (LPA) Roxana Lopez conducted an unannounced Case Management inspection due to 2 incidents that were reported to the Department on 09/18/2023, and 02/02/2024. A COVID 19 risk assessment was conducted. LPA met with Prinicipal Sal Sandoval who guided LPA on a tour of the facility. Census was taken.

On September 18th, 2023, an incident was self reported to the Department via Email by the facility who reported a staff alleged that a child's personal rights were violated while in care.



On February 2nd, 2024, one incident was self reported to the Department via Email by the facility who reported a child sustained a cut lip while in care.

All reports were reported within the required 24 hours. The purpose of the inspection was to obtain additional information regarding the incidents reported to the Department.

During the inspection, LPA Lopez conducted interviews with staff. Regarding first incident no disclosures were made on this date regarding personal rights violations. LPA asked for copies of declarations- per Principal he will email the documents to LPA. Regarding second incident first aid was administered, child was taken to the doctor for a follow up. No stiches were needed and no restrictions were given.

At this time, the licensee is in compliance with California Code of Regulations Title 22. No deficiencies cited.

A notice of site visit was given and must remain posted for 30 days.
