On Friday, October 8, 2021 at 11:24 AM, Licensing Program Analyst (LPA) Mayra Rivera entered the facility to conduct an unannounced annual inspection and met with administrator Rocio Bach who guided LPA Rivera on a tour of the facility. Facility operation hours are Monday to Friday from 7:30 AM to 6:00 PM.

At approximately 11:30 AM, LPA Rivera interviewed Rocio Bach and asked the 12 set of questions listed on the inspection tool in regards best practices in the areas of personal rights, family engagement and communication, food services, drinking water, plan of operation, teacher ratio, and personnel requirements. Rocio provided the information and LPA input the information under the section staff interview questions.

At approximately 12:23 PM LPA reviewed staff files and issued LIC 859 Staff Record Review and also reviewed children's records and issued LIC 857 Children's Records Review. Staff #1 AB 1207 Child Abuse Mandated Certification expired on 7-19-21. Staff # 2 and # 3 files are current and up to date.

At approximately 12:55 PM LPA Rivera entered the school age classroom to inspect for safety, comfort, cleanliness, and ventilation. During the inspection LPA Rivera did no observe any children present. The following staff were present during this visit: Staff #1, Staff #2, and Staff #3. LPA observed the disinfectants, cleaning solutions, and other items that are dangerous to children stored inside a file cabinet located inside the wall storage cabinet making it inaccessible to children. LPA observed cubbies for children to store their personal belongings. LPA observed furniture and equipment to be in good condition, age appropriate and free of sharp, loose, or pointed parts. For ventilation LPA observed central AC/heater vents high on the wall making it inaccessible to children to reach and temperature set at 72 degrees.

At approximately 1:06 PM, LPA Rivera entered the restroom and observed 4 toilets, 1 urinal and 4 sinks, paper towels, and hand soap dispenser full. LPA advised to place trash bins with tight fitted lids in the restroom to discard waste.

Drinking water is readily available both indoors and outdoors. LPA observed a water dispenser in the classroom and each child have their own 16 oz water bottle filled with water and staff refills the water bottle as needed. Facility provides meals and LPA observed the kitchen food preparation area to be clean and free of litter, rubbish, rodents and vermin. LPA observed the monthly menu posted on the entrance bulletin board.

Smoke detectors and carbon monoxide are present in the classroom. LPA observed the carbon monoxide detector near the entrance and heard the sound of the detector. LPA did not test the smoke alarm due to the alarm being wired and connected with the pull-down fire alarm. LPA observed two required 2A10BC fire extinguisher located in the classroom and the valve on the green area indicating fully charged and serviced on 8-19-21. First aid kits are inside the classroom and LPA observed the drill log and last drill conduct on 9-10-21.

At approximately 12:03 PM, LPA Rivera inspected the outdoor area for safety, comfort and cleanliness. The facility was observed to be free of flies, insects and rodents. LPA observed the playground fenced and locked with a padlock key. LPA observed the outdoor playground equipment in safe condition, free of sharp, lose or pointed parts. LPA did not observe climbing equipment nor slides. LPA observed sail shade triangles in place and provides adequate shade in the yard.

At approximately 12:22 PM LPA observed on the bulletin wall the license, Parents Right's, Personal Rights, LIC 610 Emergency Disaster Plan, Facility Sketch, LIC 9148 Earthquake Checklist, PUB 269 Child Passenger System Poster, monthly menu and daily schedule. LPA also reviewed the children's roaster and the sign in/out sheets. The sign in sheet for today's visit had no teacher signatures due to no children being present during the visit.

Incidental Medical Services (IMS)

Incidental Medical Services (IMS) policy was discussed. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. Criminal Record Statement

Facility representative Rocio Bach was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

LPA observed Staff #1 AB 1207 Child Abuse Certification expired on 7/19/21 and informed to complete the certification and that the mandated reporter training must be completed every 2 years, and is available atwww.mandatedreporterca.com

LPA Rivera provided technical violation for expired AB 1207 Child Abuse Certification and has given two weeks to complete the training with due date 10/25/21.
