On 3/28/24 at 11:30 AM Licensing Program Analysts (LPA) Claudia Kam and Katrina Chicote conducted an Unannounced Case management Inspection to follow up on an incidents that were reported to the Department on 3/4/24 and 3/8/24. Upon arrival, LPAs announced purpose of inspection and met with Facility Representative Director Elvia Rodriguez, who granted entry to facility. Census was taken, census at time of visit is 17.

On 3/4/24 and 3/8/24 incidents were reported to The Department where children sustained an injury while in care. The facility reported this incident to the Department within the required 24 hours and submitted written report within seven days.

On 3/4/24 Director reported an incident on 3/1/24 resulting in injury requiring medical care. During this inspection LPAs interviewed staff and director, observed room incident took place in, child was not in attendance. Teacher informed Licensing that the child was doing an activity of stringing beads one on one with the teacher in the preschool class and a bead fell. Afterwards the child requested to go home but, did not wish to disclose why she wished to go home. Another child informed the teacher the child had placed a bead up her nose. Parent and paramedics were called. Medical response was provided. Per Director, parent took the child to the doctor and was cleared to return the next day. Beads were disposed of and are no longer used in the classroom. Based on the information obtained and LPAs observation LPAs determined that there was adequate supervision and Facility Representatives made necessary adjustments after incident.

On 3/8/24 Director had reported an incident on 3/7/24 regarding an incident resulting in injury. The child had been playing and hit his head resulting in a laceration requiring medical care. LPAs spoke with staff and director regarding the incident, observed the playground equipment where the incident took place.

Staff has reported that the child was observed playing and went under the tunnel of the play structure and upon coming out was rubbing his head and teacher followed up with child and observed injury to the child's skull. Staff immediately called the parent and administered first aid by cleaning the wound and applying a bandage. Parent took child to the doctor where one staple was used to suture the wound. Child was cleared to return to care and returned the next day. Play structure was inspected by the director after the incident and it was fixed by turning the tunnel attachment upside down as the injury was observed to have small punctures caused by the coating that had dripped at time of manufacture. The tunnel is now smooth on the bottom and observed to be safe for play.

The facility was found in compliance per Title 22 regulations, there will be no deficiencies cited today.
A notice of site visit was given and must remain posted for 30 days.

Exit interview was conducted and report was reviewed with the Director Elvia Rodriguez .
