25 | On 03/07/2024 at 2:10 PM Licensing Program Analyst (LPA) Katrina Chicote conducted an Unannounced Case Management Inspection to follow up on an incident that was reported to The Department on 01/04/2024. Upon arrival, LPA met with Facility Representative (FR) Rhonda Granados, Principal, who provided LPA a guided tour of the facility inside and outside. Census was taken.
On 01/04/2024, an incident was reported to The Department in regards to construction to the exterior and interior of the facility. The facility reported this incident to the Department within the required 24 hours and submitted written report within seven days of incident per Title 22 Regulation requirements.
During this inspection LPA interviewed FR, who reported incident, and states that construction is on Phase 2 which includes interior construction to ADA update the adult and one children restroom. LPA observed indoor construction area to be completely boarded up with plywood making any hazards inaccessible to children in care at time of inspection. LPA observed there are still two additional restrooms available for children to use that includes three toilets, two urinals (equal to one toilet), and four sinks. Per FR, indoor construction is limited from 2:45 PM to 11:00 PM in efforts to limit construction during normal operating hours. At 2:45 PM, RM-1 moves to RM-2 in order to allow for construction to begin. LPA observed exterior construction to be playground on one side of the yard. LPA observed there is one outdoor play area available for children that was completed during Phase 1 of construction. LPA observed outdoor play area to have low play structure, cushioning material below that absorbs fall to be maintained, turf grass area, and a variety of age appropriate toys free of hazards at time of inspection. FR states that they are currently scheduling staggered play time for the use out this Outdoor Play Area. LPA observed other Outdoor Play Area to be completely under construction with surface to be exposed awaiting for concrete to be poured, dirt, and other construction material to be present. LPA observed this area of outdoor play area to be completely fenced off making it inaccessible to children. LPA took photos of all areas under construction at time of inspection and Outdoor Play Area that has been completed. Per FR, construction is set to conclude on 05/15/2024 and was given projected completion date prior to that. Per FR, she is currently limiting enrollment at this time to 75% of capacity to accommodate for space during ongoing construction. Report Continues - Page 1 of 2 |
