25 | On 03/07/2025 at 12:05 PM Licensing Program Analysts (LPAs) Claudia Kam and Christine Nolan conducted an Unannounced Case Management Inspection to follow up on incidents that was reported to The Department on 2/12/2025, 2/27/2025. Upon arrival, LPAs met with Facility Representative (FR) Rhonda Granados, Principal, who provided LPAs a guided tour of the facility inside and outside. Census was taken.
On 2/12/2025 and 2/27/2025, incidents was reported to The Department in regards to injury requiring medical and supervision. The facility reported this incident to the Department within the required 24 hours and submitted written report within seven days of incident per Title 22 Regulation requirements.
During this inspection LPAs interviewed FR, who reported incidents, and states that she observed children with no supervision during transition on 2/12/25. LPAs observed playground where child was injured requiring medical on 2/27/25. LPAs observed a supervision map in place for outdoor supervision of children. Child has returned to care after injury. LPAs observed outdoor play area to have cushioning material below that absorbs fall to be maintained, turf grass area, and a variety of age appropriate toys free of hazards at time of inspection.Based on LPAs observations and interview from today's inspection FR has made adequate response and action plan for supervision of children both indoors and outdoors.
Based on this information the following deficiencies on the LIC 809 D are being cited today 3/7/2025.
A notice of site visit was given and must remain posted for 30 days.
