25 | On 4/02/25 Licensing Program Analyst (LPA) Ranita Richmond and Chartice Johnson conducted an unannounced Case Management – Incident inspection related to a self-reported Unusual Incident Report (UIR) submitted on 01/30/2025. Upon arrival, LPA met with Felicia Torrence, EEC Principal and explained the purpose of the visit. LPA observed 45 children being cared for and supervised by 10 staff.
According to the UIR, on 1/30/2023, (Teacher) grabbed a student (Child) by both arms, lifted him off his feet into the air and sat him down forcefully into a chair and yelled "sit down".
Teachers witnessed incident and notified Principal Torrence. Principal initiated internal investigation.
During today's visit LPA toured the facility and interviewed Principal Torrence.
The purpose of today's visit is to issue a citation for incident reported on 01/30/25. Based on interviews conducted by LPA which have been completed, documented, and recorded. A child was grabbed by a teacher, placed in a chair, and restrained which poses an immediate health, safety or personal rights risk to persons in care. Type A citation cited. See LIC 809D.
LPA Ranita Richmond informed Principal Felicia Torrence that this report dated 4/02/25 documents one Type A citation which shall be posted for 30 consecutive days as there is an immediate risk to the health, safety, or personal rights of children in care.
Also, LPA Richmond informed Principal Torrence to provide a copy of this licensing report dated 4/02/25 that documents any Type A citation to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.
An exit interview was conducted, a copy of this report and appeal rights was read and provided Principal Torrence.
Notice of Site Visit was provided and required to be posted for 30 days.
