Licensing Program Analyst (LPA) Lilia Hernandez conducted an unannounced inspection to the above facility on 06/14/2024. LPA arrived at the facility at 1:23PM, identified self and met with Rebecca Douglas, Designee, who guided analyst on a tour of the facility. There were 39 children and 16 staff upon arrival.

The purpose for the inspection is to follow up on a recent alteration to the facility's outdoor play yard. On 05/14/2024, LPA was notified that matting under the climbing structure was going to be repaired or replaced if needed.

The facility reported to the department on 05/31/2024 that alterations were complete. The facility submitted photos of the new matting installed. The facility also submitted a letter from the Office of Environmental Health and Safety, an internal LAUSD district department, stating that an inspection was conducted and matting was adequately installed and no tripping hazards were noted.

LPA observed the surface of the outdoor play yard to be in a safe condition and free of hazards. Per Designee, all areas around and under the climbing equipment was replaced with cushioned padding that will absorb falls.

LPA advised that no children shall be left without the supervision of a teacher at any time

There were no deficiencies cited during today’s inspection.

A notice of site visit was given and must remain posted for 30 days.

Exit interview conducted and report was reviewed with Rebecca Douglas, Designee.
