Licensing Program Analyst (LPA) Lilia Hernandez conducted an unannounced case management inspection on 09/10/2024 due to a self reported incident that occurred at the facility. LPA arrived at the facility at 12:20PM and met with Jennifer Maddocks, Principal. There were 62 children and 13 staff present upon arrival.

The incident that was reported to the facility on 08/27/2024 occurred on 08/26/2024, and was reported to the Department on 08/27/2024, via email. The facility reported the Unusual Incident to the Department within the required 24 hours of occurrence.
