On 07/06/2023 at 08:15 AM Licensing Program Analyst (LPA) Silva Garibyan conducted an unannounced visit for the purpose of an increase of capacity inspection for 8219 Quartz Avenue, Winnetka, CA 91306. Home is located on a lot with a front house with address: 8221 Quartz Ave, Winnetka, CA 91306. Licensee resides in the back house with her mother and her minor daughter. The front house will not have access to the Licensee's home and yard area which is separated by a wooden fence. Parents will enter the front gate and walk to the left side of the driveway to enter the facility.

The purpose of this increase of capacity visit is to ensure the standards for a Family Child Care Home are being met in accordance to California Tittle 22 Regulations and California Health and Safety Codes. The facility has been licensed for Small (8) Family Child Care home license since September of 2022. Licensee was previously licensed as a Small Family Child Care Home under facility # 197494454. The licensee is applying for an increase of capacity; Large Family Child Care Home license for a maximum capacity of 14. The Fire clearance was granted on 06/13/2023 by LA City Fire Prevention Bureau (inspector Cindy Struck). The licensee has installed the pull fire alarm that is required for large Family Child Care Homes. Present during today's inspection was Licensee and licensee's mother. Hours of operation for the facility will be 7:00 am - 6:00 pm Monday- Friday and will be closed all major holidays. Ages served are 3 months - 7 years old. Page 1 of 5

Family members residing in the home include the licensee, licensee's mother, and licensee's minor daughter (7 years old). Licensee was informed that her children under 10 years of age, count in her capacity whenever they are present.

Licensee rents the home and provided proof of control of property by Rental Agreement And/Or Lease.

Because the licensee rents the home, proof of landlord notification is required. The LPA observed the Property Owner/Landlord Notification form (LIC9151) that the applicant confirms was provided to the property owner/landlord. The applicant obtained a signed Property Owner/Landlord Consent form (LIC 9149). Licensee has submitted a disaster plan.



The licensee has completed preventive health and safety/Childhood Nutrition/ Lead Exposure Prevention and Mandated Reporter training ( completed on 06/30/22). The licensee has current pediatric CPR/First Aid training completed in September of 2022.
This is a single story dwelling with two bedrooms, two bathrooms, kitchen, and laundry area.
Areas that are accessible to children are as follows: Living room, kitchen, bathroom #1, front yard and right side play yard. The off-limits areas are as follows: Bedroom #1, #2, bathroom #2 located inside bedroom #2. The laundry area, backyard and left side yard and front house is also off limits. Off-limits made inaccessible by use of safety door knobs and safety gates.

The home was inspected inside and out for safety, comfort, cleanliness, telephone service, heating and ventilation, poisons, detergents/cleaning compounds, medication and hazardous items that can pose a danger to children are inaccessible.

LPA observed age appropriate safe toys inside and outside. LPA observed 10 mats, a crib and a changing table in the living room. Licensee has a smoke detector and carbon monoxide that was tested and operating. LPA observed Fire Extinguisher. Medications and knives are stored in upper kitchen inaccessible to children. Cleaning products are kept under the sink inaccessible to children.

Licensee has the required documents posted in the FCCH: Facility License (LIC 203), Emergency Disaster Plan (LIC610A), Notification of Parents' Rights Poster (PUB 394), If You see Something Say Something poster, Car Seat Safety poster. Child Care Facility Roster (LIC9040) was on file. LPA reviewed children's files.


The home was found to be clean and orderly with proper ventilation for safety and comfort. The bathroom was inspected for inaccessibility of chemicals/toxins and other potential hazards to children in care. The Fire Extinguisher (2-A:10-B:C) is mounted on the wall in the kitchen. There is a working smoke/Carbon Monoxide detectors located in the kitchen.

LPA observed toys and furniture that were age appropriate and in good repair. LPA toured the backyard and found it to be fully fenced. Children's outdoor play equipment and toys are age appropriate and in good repair. LPA observed two storage sheds in the backyard off limits to children in care.

Licensee was reminded that all adults 18 and over living in the home, persons who provide care and supervision to children, and staff who have contact with children, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5-days or, if the penalty is for a repeat violation, for a maximum of 30-days per person will be assessed if this regulation is violated.

Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02-CCP. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. LPA discussed the safe sleep regulations with [applicant, licensee, or facility representative] and discussed the Child Care Licensing Safe Sleep webpage at: https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep, as an additional resource. LPA also informed [applicant, licensee, or facility representative] of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at: https://www.cpsc.gov/, and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

LPA reviewed with licensee the LIC 311D, Forms/Records to Keep In Your Family Child Care Homes, children’s forms/records, facility forms/records, and information to be posted.
On this date, 05/02/2023, the California Attorney General - Megan’s Law website was searched for information on sex offenders required to register with local law enforcement under California's Megan's Law. No registered sex offenders were found at the facility addresses. Under state law, some registered sex offenders are not subject to public disclosure; therefore, they may not have been included in this search. However, the Department conducts a monthly cross reference of each address on record for all registered sex offenders against all CCLD facility addresses pursuant to information shared by California DOJ.
Licensee was informed of the MyChildCarePlan.org site, a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.
A notice of site visit was given to licensee and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days.
Exit interview conducted and report was reviewed with the licensee, Maria Poladian.
