THIS IS AN AMENDED REPORT THAT SUPERSEDES THE PREVIOUS DOCUMENT PROVIDED CHANGES WERE MADE TO REFLECT THE PRESCHOOL INSPECTION

On 08/24/2023 Licensing Program Analysts (LPAs) Laticia Thompson and Jillinda Chandler met with Applicant Roshawn Hooper. The purpose of the visit was an announced pre-licensing inspection at Fairview Christian Academy & Enrichment Center. The applicant is requesting a license for a capacity of 60 Preschool children ages 2 - 5 years. The childcare center will operate Monday – Sunday 5:00 a.m. – 8:00 p.m. There is an approved fire clearance conducted by Walter Bergman of the L.A. City fire department. The Applicant has pending applications on file for School Age Program (197500099) and An Infant Program (195700097). The facility is located on the Angeles Mesa Presbyterian Church premises. The church is a two story building and day care operation shall be conducted on the lower level only.

LPA observed the following of the indoor activity space:


Age-appropriate furniture and equipment was observed in good repair
The room was equipped with central heating, fans and windows for ventilation.
Fire extinguishers were last serviced on 4/21/2023.
Carbon monoxide detector were present and operable during today’s inspection.
Waste bins has tight fitting lids.
LPA’s observed 2 refrigerators for storage of all foods or beverages that can cause food infections or food intoxications. Applicant will ensure the appliances will be maintain at a temperature of 45 degrees F (7.2 degrees C) or less. Applicant shall provide a thermostat for the refrigeration.

Applicant was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5-days or, if the penalty is for a repeat violation, for a maximum of 30-days per person will be assessed if this regulation is violated.

Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02-CCP. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. LPA reviewed with applicant the LIC 311A, Records to Be Maintained At The Facility, for child’s records, personnel records, administrative records, and documents to be posted.
LPA discussed the safe sleep regulations with applicant and discussed the Child Care Licensing Safe Sleep webpage at: https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep, as an additional resource. LPA also informed [applicant, licensee, or facility representative] of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at: https://www.cpsc.gov/, and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment

First aid containing bandages, tweezers, thermometer, and scissors were observed.


A parent board with the required posting are in a common area for viewing.
Individual cubbies for children belongings were observed.

Indoor total measurements: 1441.10 (sq) feet (ft) divided by 35 sq ft = 41 children.

Food services: The applicant will provide meals and snacks. Per the applicant the main kitchen will not be used for food prepping. LPAs observed vermin feces in the cabinets. Applicant shall provide extermination reports prior to licensure.

There are 3 restrooms dedicated to the Preschool program. One restroom is located four doors down from the school age program the left side of the corridor, another restroom is located in between classrooms 2 and 3. There is a restroom located near the office and lobby entrance of the facility on the right hand side.
There is a total of

3 Sinks = 45

Outdoor activity space:
The Facility is requesting a waiver to share outdoor activity space between Preschool and School-Age
The play yard was enclosed by a four feet or higher gate.
Toys and equipment were age appropriate in fair condition.
The play area is an asphalt turf. Applicant shall remove all debris and supplies from the outdoor activity area.
Applicant shall pad all poles and basketball stands.
Applicant shall devise a plan to make open railing near the entrance to the play yard, inaccessible to children in care

A shaded bench area was observed, filtered water will be provided during outdoor activities.--Page 2 of 4
