Licensing Program Analyst (LPA) Dawn Dowling and Licensing Program Manager (LPM) Raul Navarro conducted an unannounced complaint inspection on10/21/2025 at 11:50 am to deliver findings on the above complaint. LPA and LPM met with Roshawn Hooper. There were 46 children in care. Per Director, the hours of operation are Monday through Friday from 6:00 am to 6:00 pm.

The complaint investigation was conducted by Investigations Branch (IB) Investigator Jorge Rojas.
During the course of the investigation, several interviews were conducted with facility staff, the facility owner, the reporting party, and Staff#1, Medical records, the police report, and facility surveillance of the incident were obtained and reviewed. After reviewing the facility surveillance video, it was determined that Staff #1 initiated the altercation, however, Staff #2 fought back and then used a knife to stab Staff #1 multiple times. Staff #1 admitted to throwing the first punch. Several witnesses were able to corroborate the events that occurred. The violent altercation between Staff#1 and Staff# 2 posed a risk to children and other staff; therefore, the allegation of conduct inimical is substantiated.

Per Licensee, both staff were terminated as of 06/26/2025. Licensee also stated that they had staff meeting, hired security that walks along the perimeters, doing bag checks, empty pockets when they come in, making sure no contraband just to ensure safety.

The Type A deficiency listed on the attached LIC 809D page is being cited.

LPA Dowling informed the Director that Type A citation(s) shall be posted for 30 consecutive days as there are immediate risk(s) to the health, safety, or personal rights of children in care.

LPA Dowling also informed the Director to provide a copy of this licensing report that documents a Type A citation to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report.

A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.
A copy of this report, appeal rights, and Notice of Site Visit was provided.

The Notice of Site Visit must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00.
