Licensing Program Analysts (LPAs) Joe Katrdzhyan and Lilia Hernandez conducted an announced pre licensing inspection on 11/20/2023. The purpose of the visit was to review a change in ownership application at the facility to ensure that health, safety and personal rights as required by Title 22 and Health and Safety Regulations governing California Child Care Centers will be met. There were 7 children present during this inspection.

LPAs met with Arbi Delkeoukian, Applicant and Aasritha Puram, Director, who assisted LPAs with touring the facility.

The facility is currently licensed as a profit corporation, License # 197420033.

This is a proposed Pre-School Program which consists of one large room, director's office, break room, and food preparation area. This program will serve children ages, 2 years to 5 years. This program will operate Monday - Friday 7:00 AM - 6:00 PM.

All areas identified on the facility sketch were toured both indoors and outdoors; measurements were taken.
Sign-In and Sign-Out books are located in the office and a health check will be conducted prior to children signing in on a daily basis. There is drinking water readily available to children.

Furniture and equipment were inspected for age appropriateness and good repair. Telephone service, heating, lighting and ventilation were evaluated. Children have their own cubby to store their belongings. Linen will be taken home each week to be washed. Napping equipment (cots) were observed in a separate storage room. ---Page 1 of 5

Per Applicant, the isolation area is located near the office. Age appropriate sinks and toilets were inspected for availability and good repair in all restrooms. LPAs observed that facility does have a carbon monoxide detector, smoke detector, and a serviced fire extinguisher.

Disinfectants, cleaning solutions, and other items that are dangerous to children, will be inaccessible to children and stored in the staff bathroom. Director states that poisons are also locked in the staff bathroom.

Parents are required to bring their child(/ren) lunch. A weekly snack menu will be made available and posted.

All kitchen areas/food preparation areas and food storage areas were observed to be clean and are free of litter, rubbish, rodents, and/or any other vermin. All storage containers for solid waste, including moveable bins shall have tight-fitting covers that are kept on, and in good repair. Trash cans used to discard food have tight fitting lids. The facility was observed to be free of flies, other insects and rodents.

Outdoor playground equipment was inspected. LPAs observed the current outdoor emergency exit blocked with vinyl fencing. The blocked passageway on the metal fencing (behind the vinyl fencing) was observed to have a 23.5 inch clearance. The requirement per the fire department is at 36 inches.

LPA's observed loose bricks on cinder block fencing along property line next to the sand box where children play.

LPA's also observed an unstable outdoor storage cabinet next to exit door, and a dilapidated wooden playhouse with exposed metal hinges from missing door, dry brittle wood, and cracked wood inside of playhouse.

LPAs advised Applicant that the children need to be within the direct care and supervision, including visual supervision of the teacher(s) at all times.

LPAs observed a section of the walkway to be uneven and the cushioned patting to be torn that could become a tripping hazard. ---Page 2 of 5

All areas around or under high climbing equipment, swings, slides, and similar equipment are cushioned with material that absorbs a fall. There is adequate shade in the play yard. LPAs observed a tarp placed over the outdoor play area. Availability of outdoor drinking water was observed.

There are no bodies of water on the premises.

Applicant stated that medication will be stored and locked in the storage area located in the break room. First aid kits are available in each classroom and were observed to contain the required first-aid supplies. Incidental Medical Services (IMS) policy was discussed. Items discussed during this visit:
I. Licensing does not tolerate the following: Refused Entry to a Facility or Any Part of a Facility is a violation of Section 1596.852, 1596.853 or 1597.09. Regulations 101238 (g) (2), The Presence of an Excluded Individual, Fire Clearance Violations, Accessible Bodies of Water, Accessible Firearms, Ammunition or Both.
II. Breakfast/Lunch/Snack Menus: Menus are required to be posted one week in advance where it is visible by the child's authorized representative. Menus for the past 30 days must also be available upon request.
III. Current Children’s Roster: Each child day care facility shall maintain a current roster of children who are provided care in the facility. The roster shall include the name, address, and daytime telephone number of the child's parent or guardian, and the name and telephone number of the child's physician. This roster shall be available to the licensing agency upon request.
IV. Pediatric First Aid and CPR: At least one person trained in Pediatric First Aid and CPR must be present. Per applicant, Teacher’s will have a copy of their teacher permit available.
V. Designated Staff: The name of the child care center director or fully qualified teacher(s) designated to act in the director's absence must be on file.
VI. Qualifications: Educational background, training, and/or experience for each staff present must be available for review. Per applicant, Teacher’s will have a copy of their teacher permit available.
VII. Children’s Records: must be available for review; including but not limited to, the following: Name, address and telephone number of the child's authorized representative and of relatives or others who can assume responsibility for the child if the authorized representative cannot be reached when necessary.---Page 3 of 5

VIII. Immunization Requirements: Commencing September 1, 2016, a person shall not be employed or volunteer at a day care center if he or she has not been immunized against influenza, pertussis, and measles.
IX. LIC 311A - Records to be Maintained at the Facility: Child Care Centers was provided to the applicant during this visit.
X. AB1207: The following was discussed: As of January 1, 2018, Health & Safety Code 1596.8662 requires all licensed providers, applicants, directors and employees to complete training as specified on their mandated reporter duties and to renew their training every two years. New employees shall have 90 days from date of employment to complete training as required. The training may be met by using the Department’s Office of Child Abuse Prevention (OCAP) online training modules at: http://www.mandatedreporterca.com/.

Liability insurance was also discussed.

LPA reviewed with applicant the LIC 311A, Records to Be Maintained At The Facility, for child’s records, personnel records, administrative records, and documents to be posted.
Applicant was informed of the MyChildCarePlan.org site, a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

Community Care Licensing Division (CCLD) regularly sends information to licensed facilities, providers, and stakeholders by way of Provider Information Notices (PIN), Program Quarterly Update Newsletters and other important information communication platforms.

Based on measurements taken during today’s visit, Large indoor Area has space for 30 children. Measurements for the outdoor yard has space for 17 children.

Prior to Licensing the following items are to be corrected and in compliance with Title 22, California Code of Regulations:
  • 36-inch emergency exit gate must be made accessible
  • Original signed documents for outdoor waiver.
  • Updated LIC999 for outdoor yard identifying location of emergency exit gate.
  • Repair indoor cracked flooring near front entrance door.
  • Secure unstable outdoor storage cabinet next to exit door
  • Repair loose bricks on cinder block fencing along property line
  • Remove or repair dilapidated wooden playhouse - exposed metal hinges for missing door, dry brittle wood, cracked wood inside of playhouse.
  • Repair uneven grounds

The above items are due on or before 12/20/2023

The applicant is seeking to provide care for 30 preschool children; however, after final review of application and based on measurements taken today, the maximum allowed will be 30 preschoolers. Original fire clearance was granted on 5/18/2023. However, a second fire clearance will be needed due to the current emergency exit being blocked and currently at 23.5 – inches in width.

Applicant was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5-days or, if the penalty is for a repeat violation, for a maximum of 30-days per person will be assessed if this regulation is violated.
