On 11/29/2023 Licensing Program Analysts (LPA) Laticia Thompson and Jillinda Chandler met with Applicant, Noesh Hordagoda. The purpose of the visit was an announced pre-licensing inspection at the facility to ensure that health, safety and personal rights as required by Title 22 and Health and Safety Regulations governing California Child Care Centers will be met. There were no children present during this inspection. This is an application for 23 infants ages 0-2years with a toddler option of 17 children, ages 18 months – 36 months. The child care center will operate Monday – Friday 7:00 a.m. – 6:00 p.m. The fire clearance was approved on 06/26/2023. LPA’s toured and measured the on-limits areas and classrooms located at the facility. The facility has a total of 11 classrooms. Classrooms #9, #10 and #11 will be dedicated to the infant Center. Children will have access to 2 sinks and 1 toilet. Applicant will use potty seats. The licensee shall require that each child be signed in and out by his or her representative. Parents will sign children in and out in front of classroom #11.

Classroom measurements are:

The Indoor total measurements contain a total of 1,218.099 divided by 35 square feet equals 34 children, which will not accommodate Applicant’s request for 40 children.

Outdoor Play measurements are:

The infant Outdoor activity space total measurements contain a total of 2,072.298 divided by 75 square feet, which will accommodate Applicant’s request for 23 infant children.

The toddler outdoor activity space total measurements contain a total of 1,692.23 divided by 75 square feet, which will accommodate Applicant’s request for 17 toddler children.

Indoor/Children’s Area:

Indoor activity space for infants shall be physically separate from space used by preschool children’s components.

All outdoor and indoor passageways, open porches and other areas of potential hazard were observed to be free of obstruction. The child care center was observed to be clean, safe, sanitary and in good repair. Floors of all rooms have a surface that is safe and clean. The facility has central heating and air-conditioning unit to maintain a comfortable temperature for children. The infant care center is equipped with appropriate furniture and equipment. LPA’s observed cots, 8 cribs, 4 changing tables and feeding/activity chairs. Tables and chairs were observed to meet the needs of the children. LPA’s observed furniture in good repair and safe condition. The type, height and size of furniture equipment were observed to be age appropriate. There are individual permanent and portable storage space for children clothing, personal belongings and/or bedding.

LPA advised applicant of Health and Safety Code 1597.16(a)(1) Lead Testing and PIN-21-21-CCP Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010 to test their drinking water for lead contamination between January 1, 2020 and January 1, 2023, and then every 5 years after the date of the first test. Per Written Directive 100700(c)(1) Written Directives for Lead Testing (1) For a license issued on or after July 1, 2022, initial testing results shall be received and posted within 180 days of licensure.

LPA observed filtered water dispensers in classroom #9 and #10. Applicant will provide filtered water bottles to children during outdoor play. Applicant will allow children to provide personal water bottles.

The applicant will take measures to keep the center free of flies, other insects, and rodents. LPA’s observed trash containers with tight fitting cover in good repair leakproof and rodent-proof. LPA’s did not observe any fireplaces or open-faced heaters. All window screens are in good repair and free of insects, dirt and debris.

LPA’s observed a first aid kit with a current edition of a first-aid manual, sterile first-aid dressings, bandages or roller bandages, adhesive tape, scissors, tweezers, thermometer, and antiseptic solution.

Applicant will store disinfectants, cleaning solutions, poisons and other items that could pose a danger if readily available to children will be in the preschool classroom area inside a cabinet with a safety latch. LPA’s observed a fire extinguisher located in each classroom #9 and #10 with an inspection tag dated 8/2/2023.

The applicant shall be responsible for ensuring that children with obvious symptoms of illness including, but not limited to, fever or vomiting, are not accepted.

Applicant stated the office will be used as the isolation area to isolate and care for any child who becomes ill during the day. The isolation area shall be equipped with a crib, cot, mat or playpen for each ill infant. Applicant will ensure any infant in the isolation area shall be under constant visual observation by a director, assistant director, a teacher or an aide.

There are carbon monoxide and smoke detectors in all rooms. The child care centers has a working landline telephone service on the premises.

Smoking is prohibited on the premises of a child care center as specified in Health and Safety Code Section 1596.795(b). A baby walker shall not be allowed on the premises of a child care center in accordance with Health and Safety Code Sections 1596.846(b) and (c).

Napping:

A standard size crib or porta-crib meeting the following requirements shall be provided for each infant who is unable to climb out of a crib: Stacking wall cribs or cribs stacked one on top of another, often referred to as tiered cribs, shall not be permitted. Cribs shall not limit the ability of staff to see the infant. Crib mattresses shall be: Covered


with vinyl or similar moisture-resistant material. Crib mattresses shall be: Maintained in a safe condition with no exposed foam, batting or coils. Floor mats or cots that meet the requirements of Section 101239.1(b) shall be provided for all infants who have the ability to climb out of a crib. Each crib, mat or cot shall be occupied by only one infant at a time.
The applicant will provide a variety of age-appropriate equipment, toys and materials in good condition and in sufficient quantity to allow children present to fully participate in planned activities.

Infant Changing Tables

LPA’s observed changing tables having a padded surface no less than one-inch thick and covered with washable vinyl or plastic. Have raised sides at least three inches high, maintained in good repair and safe condition. While in use, be placed within arm's reach of a sink.

Restrooms:

The facility will not use hot water temperature. LPA’s observed a restroom designated to staff located in classroom #11. Applicant will post a sign identifying staff restroom area.

Outdoor:

Outdoor activity space is physically separate for infant and toddler component.

LPA.s observed a building awnings that will provide shaded rest area for the children. Applicant provided a receipt for purchase of benches. The outodoor activity area is free of hazards including, but not limited to, holes, broken glass and other debris, and dry grasses that pose a fire hazard.

The outdoor activity space is enclosed by a fence to protect children and to keep them in the outdoor area. The fencing is at least four feet high.

LPA’s did not observed any bodies of water on the premises.

Health Related Services:

Applicant will ensure all medications shall be kept in a safe place inaccessible to children. A refrigerator shall be used to store any medication that requires refrigeration.

The applicant shall develop and implement a written plan to record the administration of prescription and nonprescription medications and to inform the child's authorized representative daily when such medications have been given.

Food Service:

Prior to the infant's first day at the center, the infant care center director or assistant director shall complete a needs and services plan for the infant. High chairs or other appropriate seating equipment shall be used to seat infants during feeding. Infants who are unable to sit unassisted in a high chair or other seating equipment shall be held by a staff person or other adult for feeding. Bottles, dishes and containers of food brought by the infant's authorized representative shall be labeled with the infant's name and the current date. The infant care center shall not serve honey or corn syrup to any infant.

The facility has a refrigerator located in classroom # 10 to store foods bottles and beverages. The refrigerator shall be maintained at 45 degrees F (7.2 degrees C) or less.

Applicant will ensure pesticides and other similar toxic substances will not be stored in food storerooms, kitchen areas, food-preparation areas, or areas where kitchen equipment or utensils are stored.

Soaps, detergents, cleaning compounds or similar substances shall be stored in areas separate from food supplies.

All kitchen, food-preparation and storage areas shall be kept clean and free of litter and rubbish; and measures shall be taken to keep all such areas free of rodents and other vermin.

The food-preparation area shall not be used for: Children's play activities, unless such activities are part of supervised food-education program. Full-day programs shall offer a midmorning and a midafternoon snack.

The infant care center shall obtain written permission from child authorized representative for placement of the child in the toddler program.

Postings:

LPA observed the following postings in a prominent, publicly accessible location in the center. LIC 613A (9/96) ,PUB 393 (8/02), Child Care Center Notification of Parents' Rights and LIC 610 Disaster Plan.

Applicant will post menus at the entrance of the classrooms where parent sign children in and out. LPA advised applicant that copies of the menus shall be dated and kept on file for at least 30 days. Menus shall be made available for review by the child's authorized representative and the Department upon request.

The applicant was advised to post signs at the entrance to the child care center that provide the telephone number of the local health department and information on child passenger restraint systems pursuant to Health and Safety Code section 1596.95(g) and Vehicle Code sections 27360 and 27360.5 FORM: LIC 610

LPA observed a disaster and mass casualty plan of action.

LPA advised the applicant to reveal each child care center license number in all advertisements in accordance with Health and Safety Code Section 1596.861

Facility does not provide transportation.

The following items were discussed:

Applicant was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5-days or, if the penalty is for a repeat violation, for a maximum of 30-days per person will be assessed if this regulation is violated.

LPA discussed the safe sleep regulations with applicant and discussed the Child Care Licensing Safe Sleep webpage at: https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep, as an additional resource. LPA also informed applicant of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at: https://www.cpsc.gov/, and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

The following corrections are needed before a licensure will be considered. Corrections are due by : December 15, 2023.

· Cover metal fixture in infant play area

· Cover wood paneling with splintering in toddler outdoor activity space.

· Provide cushioning on fixtures in toddler outdoor activity space.

· Place safety covers on door handles in rooms #8, 9 and #10

· Provide pictures of Crib evacuation wheels.
