Licensing Program Analysts (LPAs) Tatiana Bickham and Decondia Ferguson conducted an unannounced site visit on 02/19/2026 at 9:10 AM to this facility to deliver findings on the above-mentioned allegations. Upon arrival, LPA met with Director Shannon Kelly, and explained the purpose of the visit. There were 2 infants being supervised by 2 staff and 5 preschool children being supervised by 1 staff at the time of visit.

During the course of the investigation, interviews were conducted with Director, parents, children, and staff. LPA conducted observations and reviewed the parent handbook. Copies of the children's and staff rosters were also obtained and reviewed.

Per Reporting Party, staff did not prevent child from being bitten multiple times by another children in care.

The Director stated that the facility changes diapers every two hours or as necessary. The Director explained that staff receive training and attend meetings on proper diaper-changing procedures and ways to prevent diaper rashes. If a child develops a diaper rash while in care, parents are notified and asked to provide diaper cream. Children are changed upon arrival if needed and before leaving. The Director stated that parents are kept informed about diaper changes for infants through Brightwheel. Diaper changes and soiled diapers are logged in Brightwheel. The Director also stated that the facility has a diapering policy included in the parent handbook.

Staff interviews were inconsistent with the Director’s statement regarding the frequency of diaper changes. Staff reported that children’s diapers are changed every hour or as needed. Staff also stated that there have been no recent cases of diaper rash. If a child develops a rash while in care, parents are notified and asked to provide diaper cream.

LPA interviewed the preschool children but the allegation does not apply to them as they are all potty trained. Infants interviewed were unable to articulate due to age.

Parents interviewed did not express concerns regarding the allegation. Some parents reported being satisfied with the services provided by the facility, while others expressed dissatisfaction.


LPA reviewed the parent handbook and did not find documentation regarding diapering or diaper-changing procedures. LPA received a copy of the diaper log from the facility but the log was missing dates and times, and LPA was unable verify how often diapers were changed. LPA also received screen shots of the diaper log from the Reporting Party, which indicated diaper changes occurred between 3–5 hours apart.


Based on the investigation conducted, there is insufficient evidence to support the above-mentioned allegations to be true. Therefore, although the allegations may have happened or are valid, there is not a
preponderance of evidence to prove the alleged violations did or did not occur, therefore the allegations are Unsubstantiated.

The Notice of Site Visit was provided and must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00.

Exit interview was conducted with Director Shannon Kelly and appeal rights were provided.

The Director stated that there was one child who had bitten another child on different occasions. According to the Director, these incidents occurred very quickly, before staff could intervene. The Director explained that the center has a policy regarding biting: if a child bites another child three times in one day, the child is sent home. When staff observe a child biting, they separate the children and explain that “we do not bite our friends.” If a child consistently bites, the facility assigns a staff member to shadow the child to prevent further incidents. Per Director, if any incidents occur parents are notified via Brightwheel.

Staff confirmed that there have been incidents where children were bitten. Staff stated that if a child bites three times in one day, the child is sent home. Staff also reported that they attempt to separate children immediately if someone is bitten.


Children stated that they enjoy attending the center. One child reported that they had been bitten while in care.

Parents interviewed did not express any concerns regarding the above mentioned allegation. Some parents stated they are satisfied with the services provided by the facility, while others expressed dissatisfaction.


LPAs did not observe any biting incidents during the visit. All children were observed to be properly supervised.

LPAs reviewed the parent handbook and did not find any documentation regarding a biting policy.


Based on LPA’s interviews, the allegation of staff did not prevent child from being bitten multiple times by another children in care. is Substantiated. Meaning the preponderance of evidence standard has been met, therefore the above allegation is found to be Substantiated. California Code of Regulations, (Title 22), are being cited on the attached LIC 9099D.

Exit interview was conducted with Director Shannon Kelly. Appeals Rights and the Notice of Site visit were provided. The Notice of Site Visit must remain posted for 30 days during the hours of operation. Failure to maintain posting as required will result in a civil penalty of $100.00.
