Per Reporting party, Staff yells and call children bad name.
One child disclosed that Staff #3 called them "stupid," which caused the child to cry and be isolated in a room. All staff interviewed denied yelling or using inappropriate language with children.
Pertaining to the allegation "Provider forces children to eat".
Per reporting party staff forces the children to eat even though they’re full.
Two of six children interviewed stated they are made to eat soup despite being full and expressing dislike for it. Staff denied forcing children to eat, with one stating that soup is offered for its nutritional value and children are not required to eat if they choose not to.
No disclosures were made from the parents. All parents interviewed stated they are happy with the service the home provides.
Based upon interviews conducted, the allegations "Provider communicates inappropriately to children in care" and "Provider forces children to eat" have been determined to be Substantiated. A finding that the complaint is substantiated means that the allegation is valid because the preponderance of the evidence standards has been met. A Type A violation is being issued as a result of today’s visit.
LPA Jeanine Lipsey informed licensee representative, Seva Mirzatagi, that this report dated 12/2/25 documents One Type A citation which shall be posted for 30 consecutive days as there is immediate risk to the health, Safety, or personal rights of children in care.
Also, LPA Jeanine Lipsey informed the licensee to provide a copy of this licensing report dated 12/2/25 that documents any Type A citation to parents/guardian of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC9924), or written statement, must be placed in the child's file for verification.
An exit interview was conducted, a copy of this report and notice of site visit were provided to Seva Mirzatagi, and was advised the notice of site visit must remain posted for 30 days. |
