This is an amendment of an original report created on 12/02/25. The report has been amended to change the 9099D page.
On 12/2/2025 at 10:52AM, Licensing Program Analyst (LPA) Jeanine Lipsey conducted an unannounced complaint investigation visit to deliver findings on the above-mentioned allegation. LPA met with Staff Seva Mirzatagi who lead LPA on a tour of the facility. LPA toured the home and observed 6 children in care being supervised by 2 adults. Licensee Ajas Nabiyev was not present on todays visits. Seva admitted she oversees the childcare and her son only has it because of a personal reason. Seva stated they are waiting for the ok to take over the childcare again on paper. LPA thanked Seva for admitting to taking care of the children. Licensee was called and stated via telephone, they were out of of the country on an emergency and left on 12/2/25 and will return 12/9/25 to visit ill family and for a personal reason. LPA advised Licensee that the department was not aware of his departure and that he must be here during child care hours.
Throughout the course of the investigation, LPA obtained the Child Care Facility Roster, interviewed parents, children, and staff.
