LPA arrived at the facility at 09:30AM, identified self and met with Yevgine Hovhannisyan, applicant, who guided analyst on a tour of the facility. LPA provided Licensee with a copy of the Entrance Checklist to help facilitate the inspection. LPA observed 0 adults and 0 children in care upon arrival. Per Licensee, operation hours are 06:00 AM to 11:00 PM, Monday to Friday. Applicant will be caring for children ages 2 to 12 years of age. This is a one-story home which consists of 3 bedrooms, 2 bathrooms, kitchen, dining room, living room, front yard, and backyard (fenced). The children use the bathroom in the main hallway. Per Licensee, areas off limits to children and parents include: all bedrooms, 1 bathroom (connected to master bedroom), and the kitchen (kitchen entrances were observed to be barricaded by mesh gates for children under the age of 5). The licensee provides food for children in care. Individuals who reside in the home were noted and discussed. Applicant states that there are no firearms stored in the home. All areas identified on the facility sketch that are accessible for children to use were inspected for safety, comfort, and cleanliness. There is telephone service via a cellphone that is used, and the cellphone stays at the facility during operation hours. There is ventilation and heating screen in hallway on the wall. Safe toys play equipment and materials were observed. Detergents, cleaning compounds, medications, and other items which could pose a danger to children were observed to be inaccessible to children. Poisons are locked in a cabinet in the restroom. The restroom that children use was observed to be safe and sanitary. --- PAGE 1

The valve on the required 2A 10BC fire extinguisher indicates fully charged and was serviced in March 2023, as indicated on service tag. Smoke and carbon monoxide detectors were tested and are operable.

Applicant states that she is not currently caring for infants.

Due to Licensee operating from 6:00 AM to 11:00 PM, LPA discussed sleeping and napping arrangements for children in care.

Children will use the back yard for outdoor play time. The outdoor play area was observed to be fenced. LPA observed that the outdoor yard has no toys and other materials for children to play with. LPA stated that the applicant needs to provide safe outdoor equipment for children. The applicant states that supervision is always provided. LPA observed stone stairs that leads down into backyard. There is a pathway on opposite side near the utility shut off (that is barricaded) that leads to backyard. An extended brick patio connects to the pathway area. LPA stated that applicant needs to provide a protecting fence for rock wall.
The applicant and other personnel have completed training on preventive health practices including Pediatric First Aid and CPR. The applicant's Pediatric First Aid and CPR expires on January 2026. There are first aid supplies available. LPA advised that if a child shows signs of illness, they shall be separated from other children. The first bedroom of the house will serve as the isolation room for sick children.

The applicant does have proof of immunization against influenza, pertussis, and measles.

LPA observed that the Licensee and assistant do/do not have proof of the Mandated Reporter AB 1207 compliant Child Care Training Certificate on file.

LPA informed applicant that all homes shall conduct fire and disaster drills at least once every six months, and document the date and time of each drill.

There are no pets on the premises.

LPA did/did not observe any pools, spas, hot tubs, fish ponds, or similar bodies of water during the inspection.

Smoking is prohibited in a licensed Family Child Care Home. Per Licensee, no one smokes in the home.

LPA has requested the following corrections for the family childcare home. Applicant must provide pictures, receipts and documentation for equipment purchased for children inside play area and outside play area. Applicant must also send pictures of the backyard patio area barricaded with fencing for safety of children. Applicant needs to provide proof of assigned space for parent’s board. Applicant will send photo of locks on kitchen cabinets and drawers.

LPA informed applicant that after all proof of corrections have been received, the application and report will go under a final review with a Licensing Program Manager (LPM), who will determine completion to obtain license.
Because the applicant rents/leases the home, proof of landlord notification is required. The LPA observed the Property Owner/Landlord Notification form (UC9151) that the applicant confirms was provided to the property owner/landlord. The applicant obtained a signed Property Owner/Landlord Consent form (UC 9149).

Applicants was reminded that all adults 18 and over living in the home, persons who provide care and supervision to children, and staff who have contact with children, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5-days or, if the penalty is for a repeat violation, for a maximum of 30-days per person will be assessed if this regulation is violated.

LPA reviewed with applicant the LIC 311D, Forms/Records to Keep in Your Family Child Care Homes, children’s forms/records, facility forms/records, and information to be posted.

On January 2024, the California Attorney General - Megan’s Law website was searched for information on sex offenders required to register with local law enforcement under California's Megan's Law. No registered sex offenders were found at the facility addresses. Under state law, some registered sex offenders are not subject to public disclosure; therefore, they may not have been included in this search. However, the Department conducts a monthly cross reference of each address on record for all registered sex offenders against all CCLD facility addresses pursuant to information shared by California DOJ.

Applicant was informed of the MyChildCarePlan.org site, a consumer education website that helps families obtain childcare by connecting them to childcare providers and Resource and Referral Agencies (R&Rs) throughout California.

LPA discussed the safe sleep regulations with [applicant, licensee, or facility representative] and discussed the Child Care Licensing Safe Sleep webpage at:

resources/safe sleep, as an additional resource.

LPA also informed applicant of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at: https://www.cpsc.gov/, and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.
