On 03/07/2024 at 9:38AM, Licensing Program Analysts (LPA) Laticia Thompson conducted an unannounced follow up inspection that Unlicensed Care has ceased. Upon arrival, LPA conducted a surveillance of the facility and did not visually observe any staff or children. There were no cars in the parkinglot area. LPA contacted owner by phone Mujtaba Noorzay and explained the purpose of the visit was to investigate a co-complaint regarding unlicensed care.

The owner arrived at the facility at 10:09. Owner guided LPA on a tour of the facility.
LPA inspected all areas of the facility and did not observe any children or staff. LPA conducted an interview with owner.

Based on observations and an interview with the owner operation has ceased. The owners application for a childcare center license (LIC 200A) is currently pending.

An exit interview was conducted with Mujtaba Noorzay (owners) in which a copy of this report and appeal rights were provided.
