On 01/16/, beginning at 08:00 AM, Licensing Program Analyst (LPA) Elicia Calvillo conducted an announced Second Pre-Licensing inspection at 4710 Degovia Avenue, Woodland Hills, CA. 91364.

Applicant is applying for a Large Family Child Care Home, anticipating caring for 14 children from ages 1 to 5 years of age. LPA met with Liudmyla Datsenko, Applicant, who guided LPA on a tour of the inside and outside of the home. Also present was 8 children and 1 adult.

During today's visit LPA, collected pending documents from applicant.

Before Facility License approval, the application will need to reviewed by the Department Management.

Exit interview conducted and report was reviewed with Liudmyla Datsenko, Applicant.
