Licensing Program Analyst (LPA) Roberto Luque Avila conducted a pre-licensing inspection on 1/16/25. LPA arrived at the facility at 9:15 AM and met with Sargis Baghdasaryan, Applicant, who guided analyst on a tour of the facility. Per applicant, operation hours will be Monday Through Sunday, 8AM to 5PM. Applicant states that they will care for children 1 to 13 years of age. Pre-Licensing Entrance Checklist LIC9280 was provided to the applicant. This inspection was translated in Armenian.

The applicant is requesting a relocation and capacity increase from their currently licensed facility at 16322 Parthenia St. CA 91343 under facility number 195700100 to their new location at 16322 1/2 Parthenia St. CA 91343 under facility number 195700403. There is an approved fire clearance from the Los Angeles Fire Department by Inspector Ted Debs dated 11/14/24

All areas identified on the facility sketch were inspected, including but not limited to, off limit areas. This is a one story ADU that consists of 2 bedrooms, 1 restrooms, dining room/ living room/kitchen and laundry closet. The ADU has a certificate of occupancy approved by the Los Angeles Department of Building and Safety. The ADU's main entrance is located in the alleyway. Based on the Facility Sketch submitted, areas off limits to children are bedroom #1, laundry closet, and outside storage area. These areas are made inaccessible by a lock and door safety latches. Applicant states that the latch will stay latched during the hours of operation. The applicant understands that licensing staff may have access to off-limit areas during inspection visit if necessary.

Per applicant, the children will use the restroom in the hallway. Areas that will be used by children were inspected for safety, comfort, cleanliness, ventilation and heating by 3 mini split units. Toys are available for children and are age appropriate. Napping equipment includes 2 cribs and 8 cots. Applicant ordered 4 more cots during inspection. Page 1 of 5

The applicant states that they will provide food for children in care. If food is not provided and food is brought from the children’s homes; container shall be labeled with child’s name and properly stored or refrigerated.

The applicant has completed the required Health and Safety Training and Nutrition Training. There are first aid supplies available located in the kitchen. Although the applicant has completed a Pediatric CPR and First Aid training online they are required to complete this training in person.

Per Title 22 section 12416(1)(a) "In addition to any other required training... each family day care home licensee who provides care shall have at least 15 hours of training on preventive health practices. The training shall include pediatric cardiopulmonary resuscitation, pediatric first aid, recognition, management, and prevention of infectious diseases, including immunization's, and prevention of childhood injuries. The training may include training on sanitary food handling, child nutrition, emergency preparedness and evacuation, caring for children with special needs, and identification and reporting of signs and symptoms of child abuse"(e) "the training required under subdivision (a) shall not be provided by a home study course. This training may be provided through on-the-job training, workshops, or classes...."

Detergents, cleaning compounds, medicines, sharp objects and hazardous items that can pose a danger to children are inaccessible via drawer locks.

The applicant states that there are no poisons on the premises or poisons. The applicant was advised that any poisons must be locked with a key or combination lock.

The children will use the backyard for outdoor play, which was observed to be fenced and have age appropriate play equipment. Per applicant, there are no pet in the home. Applicant states there are no firearms or weapons in the home. LPA did not observe any body of water during the inspection

The valve on the 2A10BC fire extinguisher indicates fully charged, and was purchased on 10/5/24. Smoke and carbon monoxide detectors were tested and are operable. Smoking is prohibited in a licensed Family Childcare Home. Per Licensee, no one smokes in the home. Page 2 of 5

Applicant was reminded that all adults 18 and over living in the home, persons who provide care and supervision to children, and staff who have contact with children, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5-days or, if the penalty is for a repeat violation, for a maximum of 30-days per person will be assessed if this regulation is violated.

On this date, 1/16/2025, the California Attorney General - Megan’s Law website was searched for information on sex offenders required to register with local law enforcement under California's Megan's Law. No registered sex offenders were found at the facility addresses. Under state law, some registered sex offenders are not subject to public disclosure; therefore, they may not have been included in this search. However, the Department conducts a monthly cross reference of each address on record for all registered sex offenders against all CCLD facility addresses pursuant to information shared by California DOJ.

The provided provided proof of control of property via certificate of occupancy.

Applicant was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

LPA discussed the safe sleep regulations with applicant and discussed the Child Care Licensing Safe Sleep webpage at: https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep, as an additional resource. LPA also informed applicant of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at: https://www.cpsc.gov/, and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

LPA advised the applicant to sleep infants where they can be directly supervised at all times and must make a log documenting infants sleep in 15-minute intervals. Licensee was advised against sleeping infants in a separate room. Applicant states that infants will sleep in the bedroom #2 where she will be providing supervision. LPA did not observe the following items during the inspection: Infant Walkers, Johnny Jumpers, Saucer Chairs, Trampolines and/or any other item that fall into these categories are not permitted in a family childcare facility.

Incidental Medical Services (IMS) policy was discussed. For IMS information, see PIN 22-02-CCP. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. LPA advised that if a child shows signs of illness he/she/they shall be separated from other children. Per applicant when a child shows signs of illness they will be separated in the bedroom #2 and parents will be notified.

LPA reviewed with the applicant the LIC 311D, Forms/Records To Keep In Your Family Child Care Homes, children’s forms/records, facility forms/records, and information to be posted. LPA advised the applicant how to access forms, regulations and quarterly updates on the Childcare Licensing website at: www.ccld.ca.gov.

Community Care Licensing Division (CCLD) regularly sends information to licensed facilities, providers, and stakeholders by way of Provider Information Notices (PIN), Program Quarterly Update Newsletters and other important information communication platforms.

Applicant was reminded of all the terms and conditions noted on the LIC 279 – Application of a Family Childcare Home that applicant signed and agreed to. The licensee shall post the license, PUB 394, and Family Childcare Home Notification of Parents’ Rights in a prominent, publicly accessible area in the family child care home at all times children are in care.

The licensee shall report the following information the department by telephone or fax within the department's next business day to include: any change in household composition, adults moving in or out of the home, anyone living in the home who reaches his or her 18th birthday, child death, any injury that requires medical treatment, and any unusual incident or child absence that threatens the physical or emotional health or safety of any child.
