On March 12, 2026 Licensing Program Analysts (LPAs) DeCondia Ferguson and Amelia Morales conducted an announced pre-licensing inspection at the applicant’s residence. The purpose of the inspection was to evaluate compliance with Title 22, California Code of Regulations, governing Family Child Care Homes, and to determine whether the applicant meets the health, safety, and personal rights requirements for licensure.

Upon arrival, the LPAs were greeted by applicant Sona Karapetyan and granted entry into the residence. An entrance checklist was provided. This application is a change of location from previously licensed facility number #198021402. The applicant acknowledged that once the new license is issued, child care services may no longer be provided at the prior address.

It should be noted that there is an additional property/unit in the front, with a separate address. There are two addresses 1622 Keeler St., Burbank, CA 91504 and 1622 Keeler St. Unit #100., Burbank, CA. The applicant resides in the back house 1622 Keeler St. Unit #100, Burbank, CA. The Applicant has a separate entrance, from the additional unit. Applicant’s home is separated by a fence, in the backyard and retractable gates that separate the front house yard from the back.

The applicant is requesting a Large Family Child Care Home license for up to 14 children, ages 0–13 years. Fire clearance was granted by the Los Angeles City Fire Prevention Bureau on January 9, 2026. The proposed hours of operation are Monday through Friday, 7:00 a.m. to 12:00 a.m. Per applicant no overnight care will be provided. FACILITY EVALUATION REPORT California law requires a public report of each licensing visit/inspection. This report is a record for the facility and the licensing agency. This report is available for public review; therefore, care is taken not to disclose personal or confidential information. Inquiries concerning the location, maintenance, and contents of these reports may be directed to the Licensing Program Analyst or Regional Office whose address and telephone number are listed on the front of this form.

DEFICIENCIES A deficiency is an instance of noncompliance with licensing requirements, including applicable statutes, regulations, interim licensing standards, operating standards, and written directives. Applicants/ licensees must be notified in writing of all licensing deficiencies. Deficiencies are listed on the left side of this form, and the applicable licensing requirement upon which the deficiency is identified. There are two types of deficiencies:
  • Type A deficiencies are violations of licensing requirements that, if not corrected, have a direct and immediate risk to the health, safety, or personal rights of persons in care.
  • Type B deficiencies are violations of licensing requirements that, without correction, could become a risk to the health, safety, or personal rights of persons in care, a recordkeeping violation that could impact the care of said persons and/or protection of their resources, or a violation that could impact those services required to meet the needs of persons in care.

PLANS OF CORRECTION (POCs) The licensing agency is required to establish a reasonable length of time to correct a deficiency. In order to set the time, the licensing agency must take into consideration the seriousness of the violation, the number of persons in care involved, and the availability of equipment and personnel necessary to correct the violation. Applicants/licensees are requested to provide a specific plan for each violation on the right side of the form across from each deficiency. The more specific the plan, the less chance exists for any misunderstanding in setting time limits and reviewing corrections. The applicant/licensee who encounters problems beyond their control in completing the corrections within the specified time frame may request and may be granted an extension of the correction due date by the licensing agency.

CORRECTION NOTIFICATION The applicant/licensee is responsible for completing all corrections and promptly notifying the licensing agency of corrections. Applicants/licensees are advised to keep a dated copy of any correspondence sent to the licensing agency concerning corrections, or if corrections are telephoned to the licensing agency, the date, person contacted, and information given.

CIVIL PENALTIES The licensing agency is required by law to issue a Penalty Notice, when applicable, to all facilities holding a license issued by the licensing agency, or subject to licensure, except Certified Family Homes, Resource Families, and Foster Family Homes, or any governmental entity.

PENALTY NOTICE GIVEN The statement concerning civil penalties serves as a penalty notice on this Licensing Report and failure to correct cited licensing deficiencies will result in civil penalties. Applicants/ licensees are required to pay civil penalties when administrative appeals have been exhausted and in accordance with any payment arrangements made with the licensing agency.

APPEAL RIGHTS The applicant/licensee has a right without prejudice to discuss any disagreement in this report with the licensing agency concerning the proper application of licensing requirements. The applicant/ licensee may request a formal review by the licensing agency to amend or dismiss the notice of deficiency and/ or civil penalty. Requests for review shall be made in writing within 15 business days of receipt of a deficiency notification or civil penalty assessment. Licensing deficiencies may be appealed pursuant to the procedures in the LIC 9058 Applicant/Licensee Rights.

AGENCY REVIEW The licensing agency review of an appeal may be conducted based upon information provided in writing by the applicant/licensee. The applicant/licensee may request an office meeting to provide additional information. The applicant/licensee will be notified in writing of the results of the agency review within 60 business days of the date when all necessary information has been provided to the licensing agency.

EMAIL REQUIREMENT Adult Community Care Facilities, Residential Care Facilities for the Chronically Ill, and Residential Care Facilities for the Elderly are required to provide and maintain an active email address of record with the licensing agency.

primary contact number during operating hours. The LPA discussed supervision and safety considerations associated with extended evening hours of operation to ensure compliance with continuous supervision requirements.

The residence is a single-story home consisting of a front yard, two bedrooms, two bathrooms, kitchen, living room, and a fenced side yard. Side yard to be used by residence of 1622 Keeler St., Burbank, CA 91504. All areas identified on the facility sketch were inspected, including those designated as off-limits.

Off-limit areas include bedroom #2 with bathroom#2 attached, was observed to be properly barricaded and inaccessible to children. The kitchen is secured with locking mechanism on cabinet and drawers rendering it inaccessible to children in care. Cabinets and drawers are equipped with child-safety latches. Knives and sharp objects were observed to be stored inaccessible to children.

The applicant stated that meals, including breakfast, lunch, dinner, and a morning and afternoon snack, will be provided. The stove, refrigerator, sink, and food preparation areas were inspected and found to be clean and operable.

Areas accessible to children in care include the living room, bedroom #1, bathroom #1, and the fenced front yard. The outdoor play area is fully enclosed. No bodies of water were observed on the premises. The applicant stated children will be directly supervised at all times during outdoor activities and will not be left unattended.

The home was observed to be clean, orderly, and free from observed hazards. Age-appropriate toys and play materials were available. There are no pets in the home.

Napping equipment, including 7 cots, were observed. The applicant stated linens will be laundered weekly by the parents. A playpen was observed in bedroom #1. The applicant was reminded that baby walkers, bouncers, jumpers, and similar prohibited equipment shall not be used in a licensed Family Child Care Home.

Bedroom #1 is designated as the isolation area for ill children. The applicant was advised that any child exhibiting signs of illness must be separated from other children and remain under supervision until picked up by an authorized representative. If the room is used for dual purposes, appropriate separation and supervision must be maintained.

All electrical outlets were equipped with safety covers. Three combination smoke and carbon monoxide detectors were observed, tested, and found to be operable. A fully stocked First Aid kit and a 2A:10-B:C fire extinguisher (inspected on April 1, 2025) was observed mounted in the bedroom #1 and the kitchen and readily accessible.

Outdoor playground equipment is in a safe condition, free of sharp, loose or pointed parts. The surface of the outdoor activity space is maintained in a safe condition and is free of hazards. All areas around or under high climbing equipment are cushioned with material that absorbs a fall. LPAs advised that no children shall be left without the supervision of a teacher at any time.

All floors were observed to be clean and safe. All materials accessible to children were observed to be toxic-free There are no firearms stored on the premises.

The applicant completed Preventive Health and Safety, Childhood Nutrition, and Lead Exposure Prevention training on June 17, 2023; Mandated Reporter training on December 17, 2025; Pediatric CPR/First Aid training on January 4, 2026, and Family Child Care Home Orientation on May 1, 2023.



The LPA reviewed LIC 311D (Forms/Records to Keep in Your Family Child Care Home), children’s records requirements, facility record requirements and required postings. The following documents were observed posted in the entrance hallway:

The applicant was advised that the facility license must be posted upon issuance. A fire/disaster drill log must be maintained, and drills must be conducted and documented at least once every six months.

Because the applicant leases the residence, proof of landlord notification was required. LPA observed LIC 9151 (Property Owner/Landlord Notification), which the applicant confirmed was provided to the landlord, and a signed LIC 9149 (Property Owner/Landlord Consent) form.

The applicant was reminded that all adults aged 18 and older residing in the home, and any individual who provides care and supervision or has contact with children must obtain a criminal record clearance or exemption prior to initial presence in the home, pursuant to Health and Safety Code section 1596.871. Civil penalties for noncompliance were discussed.

Safe Sleep regulations were reviewed. Resources were provided, including the Child Care Licensing Safe Sleep webpage: https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-andresources/safe-sleep, as an additional resource and the United States Consumer Product Safety Commission (CPSC) website https://www.cpsc.gov/, and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment. A sample infant sleep log was provided and reviewed.

Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02CCP. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. Applicants was informed of the MyChildCarePlan.org site, a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

On this date, 3/12/2026, the California Attorney General - Megan’s Law website was searched for information on sex offenders required to register with local law enforcement under California's Megan's Law. No registered sex offenders were found at the facility addresses. Under state law, some registered sex offenders are not subject to public disclosure; therefore, they may not have been included in this search. However, the Department conducts a monthly cross reference of each address on record for all registered sex offenders against all CCLD facility addresses pursuant to information shared by California DOJ.
