25 | Licensing Program Analyst (LPA) Tatiana Bickham conducted an announced pre-licensing inspection for a large family child care at the above facility on 04/13/2026 at 9:00 AM. LPA met with Applicant Anna Garkavenko, who guided LPA on a tour of the facility. Also present during today’s inspection was the applicant’s spouse. Residing in the home is the applicant, applicant’s spouse, and applicant two minor children. This is a change of location application, applicants previous license (Garkavenko Family Child Care 195700303). There were no children present when LPA arrived. Per Applicant, hours of operation will be Monday through Friday, 8:00am to 6:00pm. Applicant states they will provide care for children 1.5 years to 5 years of age.
This is a two-story home consisting of five bedrooms, four bathrooms, kitchen, living room #1, living room #2 dining room, family room, laundry room, attached garage, front yard, and backyard. The home was inspected for safety, cleanliness, telephone service, heating and ventilation, inaccessibility to poisons, detergents, cleaning compounds, medicines, and hazardous items that can pose danger to children.
Areas used by children include: The first floor: The living room #1/dining room, family room, bathroom adjacent to the family room, and front yard. Areas off limits include: The second floor: Bedroom #1 and ensuite bathroom, bedroom #2 and ensuite bathroom, bedrooms 3-5, bathrooms, laundry room, attached garage, and backyard. Off limit areas are made inaccessible by door gate. Knives and other sharp utensils are locked in the kitchen cabinets and drawers. Cleaning products are made inaccessible by a lock.
The home is clean and orderly, there are outlet covers throughout the home. Combination smoke/carbon monoxide detectors were observed, tested and found to be operable.
