On April 15, 2025, at 10:21 A.M., Licensing Program Analyst (LPA) Joselito L. Del Mundo conducted an unannounced complaint inspection at the facility to deliver findings for the above allegation. LPA Del Mundo met with the assistant director, Priscilla Rodriguez, who granted access to the facility. LPA toured the facility and observed fifty-four children with five staff providing care and supervision.

During the investigation, facility roster and Face-To-Name Transition sheets were provided by the director. LPA requested to view the video recording for the day of the incident alleging child #1 was push down by unknown staff. LPA also requested photos of injury from the reporting party but to no avail.

Based on the confidential interviews with relevant parties, supporting documents, and the LPA observation, it was found that the allegation: staff handled child in a rough manner resulting in injury, could not be corroborated. Therefore, the allegation has been rendered unsubstantiated. No deficiencies were cited at this time.

An exit Interview was conducted, and a copy of this report was read, Appeal Rights, and a Notice of Site Visit, was discussed and left with assistant director, Priscilla Rodriguez, at the facility. Failure to maintain the posting of the Notice of Site visit will result in a $100 Civil Penalty.
