32 | Licensee disclosed that on 10/17/2023, Child #5 fell while playing outdoors. Licensee stated that Staff #3 observed Child #5 fall but did not write an injury report to provide to the parent.
During an interview with Staff #3, Staff #3 disclosed that they indeed saw Child #5 fall and quickly walked over to see if Child #5 was okay. Staff #3 did not see any injuries to Child #5 and Child #5 asked to go back to play with the other children. Staff #3 disclosed that they did not provide the parent an injury report or contact parent regarding the injury.
Per licensee, there has not been any incidents of insect problems. Licensee stated a pest control company is contracted to monitor the facility for pest. (invoices provided)
Licensee also has a company who cleans the carpets monthly. (invoices provided) Licensee also has linens for napping washed weekly.
During interviews with staff, there was no disclosures made about observing insects in the facility.
During interviews with children, there was no disclosures of any bugs or bite while in care.
During interviews with parents, there was no disclosures about concerns with insects in the facility. Parent#2 stated that at no time has their child come home with any insect bites. Parent #2 is satisfied with the care their child is receiving. Parent #8 stated that there has never been an instance where their child has come home with insect bites. Parent #8 has only had great experiences at the facility.
Based on the investigation conducted by the Department, it has been determined that although the allegations may have happened or is valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur, therefore the allegations is unsubstantiated.
The Notice of Site Visit must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00.
