On 04/10/2023 Licensing Program Analyst (LPA) Doris Whitmore made an unannounced visit for the purpose of conducting a required I year inspection. LPA met with Marlene Yacoub Site Supervisor and toured the facility indoors and outdoors. LPA observed proper care and supervision during this inspection. LPA observed a total of 45 children and 12 staff present at the time of the inspection All center staff that was present during today’s visit has criminal record clearances and are associated to the designated license number. This facility serves 3 Early Head Start Classrooms, 1 Headstart/ State Preschool, and 1 Head Start. Facility hours of operation for EHS is from 8:00a.m. to 3:00p.m., Headstart/ State Pre-school is from 8:00a.m. to 3:00p.m., and the Headstart is from 8:15a.m. to 2:30p.m.LPA observed all postings requirements for operation on the postings board LIC203A- License, LIC610A-Emergency Disaster Plan, LIC9148- Earthquake Preparedness Checklist, PUB394-Notification of Parents Rights Poster, LIC613A- Personal Rights, PUB-269- Car Seat Law, Menus, Activity Schedule, Roster. Staff Files are not located at the facility and will be reviewed at a later date and time. Staff Files are located at the Central Main office Maud Booth. LPA Whitmore reviewed a total of 10 children files. Children files were reviewed and found to be in compliance.

During this inspection, LPA observed no physical changes to the facility. LPA observed a working telephone on the premises, heating, lighting and ventilation were evaluated. Furniture and equipment were inspected for age appropriateness and good repair. Napping equipment were observed to be clean and good condition. Adequate storage for children's belongings, bathrooms facilities, and separate area for isolation and care of ill children located in the office area. All Classrooms have an igloo for water and cups. Each day the teachers write their initials with the date. All trash cans had tight lids. Documentation of current Fire/Earthquake disaster drill was observed on a monthly basis. First Aid supplies were inventoried and available. Sign in/out were observed. A review of medication policy, including properly labeled and stored in original container. During today's inspection LPA reviewed the Medication Administrations log.and Individualized Health Plan Each classroom had a 2 A10BC Fire Extinguisher or larger. Page 1

This facility provides Incidental Medical Services – IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226.The following information regarding ADA was provided: US Department of Justice (USDOJ) toll-free ADA Information Line at (800) 514-0301 (voice)/ (800) 514-0383 (TTY) and link to publication: Commonly Asked Questions about Child Care Centers and the ADA, available at: http://www.ada.gov/childqanda.htm.The facility serves breakfast, lunch, and center Per center staff, foods and snacks were delivered daily to the site from the main kitchen in North Hollywood. An inspection of cleaning and food supply storage areas was made. Food preparation area was toured for safety, cleanliness, adequately equipped and inaccessible to children in care. Food and snack items are properly stored. Outdoor area observed to be fenced. Climbing structure, slides and apparatus equipment securely anchored and has cushioned with material that absorbs falls zone. Bikes were observed in good condition. LPA observed other educational materials were available for children to utilize. Play area observed free of miscellaneous debris or hazards and inaccessibility to bodies of water. Required shade and rest areas are provided, drinking water readily available and accessible to children. LPA checked restrooms and diaper log.

LPA reminded that it is the facility responsibility to know the regulations as well as anyone who assists in providing care. Site supervisor was advised on how to access quarterly reports, forms, and regulations for Child Care online at www.cdss.ca.gov. LPA also advised to read the Child Care quarterly updates every season as the come out to stay informed of any changes or updates to the regulations. LPA informed the Child Care Advocate Program (CCAP) provide many other helpful resources to the licensees and the public. Facility may also register on CCAP website for the new quarterly report to be notified. Beginning January 1, 2018, Health and Safety Code 1596.8662 requires all licensed providers, applicants, directors and employees to complete training as specified on their mandated reporter duties and to renew their training every two years.
