On March 24, 2025, Licensing Program Analyst (LPA) Calloway, made an unannounced case management inspection to the above facility. The purpose of the case management was to follow up on a self-reported Unusual Incident Report (UIR) submitted on to the Palmdale Regional Office on March 20, 2025. LPA toured the facility with the facility representative and observed thirty-four (34) preschool, infant, and toddler day children and seven staff in active care. There were no school- aged children in care but eleven children arrived later.

The reported information involved Child 1 (C1) left unattended, alone inside the school-aged classroom. LPA conducted interviews with the relevant parties. LPA reviewed staff and children files and completed a safety inspection of the school- aged classroom there was a door that leads to the outside playground and the outside gates to the facility were observed locked.

Based on interviews, and record review, it was determined that C1 was left alone, unattended inside the classroom while the staff member (S1) went up to the front office to tell the facility representative C1 was not listening to their commands and did not want to ride on the bus to pick up the other children. The facility representative notified C1’s parents and Community Care Licensing (CCL).



Based on observation, there were no hazards observed in the class room, the playground was secure and locked, and the child was not able to leave the premises. Although the staff took appropriate action, the care and supervision violation did occur.

There is one deficiency cited during this inspection. See the LIC 809D page attached to this report.

Exit interview was conducted and a copy of this report was read, a Notice of Site Visit, and Appeal Rights were provided to Melissa Rea, Facility Representative at the facility. A Notice of Site Visit must remain posted for thirty (30) consecutive days. Failure to maintain posting will result in a $100 civil penalty.

We ordered walkie talkies for each teacher if they need assistance in the classroom or outside and I am also working on doing CSR checks for name to face checks and making sure teachers know how to use them. We completed training and I will go over disciplinary action and training with Staff that was involved who was not present for the training.

Based on interview and record review Staff 1 left Child 1 alone, unattended inside the school-aged classroom and went to the front office to talk to the facility representative which poses a potential health, safety, or personal rights risk to the persons in care.
