32 | Information provided by the Reporting Party (RP) indicates that on 05/10/2024, RP was at the facility picking up their child and walking in the hallway and came upon Child #6 and Child #7 wandering alone without a teacher.
Per Director, no disclosures were made to LPA regarding children who were left unsupervised. Per Assistant Director, staff reported to the Assistant Director of an incident that occurred on 05/10/2024 when Child #6 and Child #7 were left alone in a classroom without teachers. Teachers were taking the children from the classroom to the outside play yard.
When interviewing the staff, Staff #1 indicates that two children were left alone inside a classroom without adult supervision when staff did not account for children leaving the classroom to go to the outside play yard. Staff #2 indicates that 2 children were left in the classroom unattended without adult supervision when staff did not double check if children were in the restroom. Staff #3 indicates that two children were left in a classroom when they transitioned to the outside play yard.
When interviewing parents and children, no disclosures were made regarding the allegation.
Based on LPA’s observations and interviews which were conducted and record review, the preponderance of evidence standard has been met, therefore the above allegations is found to be SUBSTANTIATED. California Code of Regulations, Title 22, Division 12 Chapter 3, Section number 101229 (a) (1) Responsibility for Providing Care and Supervision, is being cited on the attached deficiencies page.
The Notice of Site Visit (LIC 9213) must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00.
