On 11/13/2025, LPA Ranita Richmond conducted an unannounced visit to deliver the findings on the above mentioned allegations. LPA Richmond was greeted by Gabriela Osoy. LPA Richmond toured the home inside and outside for Health & Safety inspection. LPA Richmond observed 5 children being supervised and cared for by fingerprint cleared staff. Licensee and adult daughter arrived at 11:15am.

On 8/28/25 El Segundo Child Care Regional Office received a complaint regarding license, personal rights, and record keeping violations for the above mentioned facility.

A full investigation was conducted which included observations, records reviews, and interviews. Based on the observations, interviews, and record reviews, which were conducted and recorded, the preponderance of evidence standard has been met, therefore the above allegations are found to be SUBSTANTIATED. California Code of Regulations, Title 22 Division 12, Chapter 1, and Article 06, are being cited on the

On 8/19/25, Licensing Program Analyst (LPA) Ranita Richmond contacted and interviewed the Reporting Party.

On 8/28/2025, Licensing Program Analyst (LPA) Ranita Richmond, Doris Whitmore, and Lisa Clayton completed an initial 10-day complaint investigation. During inspection, the LPAs toured the facility, completed observations, obtained pertinent documents, and completed interviews with staff and a parent.

Between 08/29/25 and 11/13/25 LPA Richmond contacted parents of children enrolled in the above-mentioned Family Childcare Home to conduct interviews.

Per Title 22 Regulations and Health and Safety Codes, two (2) Type B citations are cited. See LIC 9099D.

LPA Richmond informed licensee Lisa Worthen that this report dated 11/13/25 documents one(1) Type A citation which shall be posted for 30 consecutive days as there is an immediate risk to the health, safety, or personal rights of children in care.

Also, LPA Richmond informed the licensee Lisa Worthen to provide a copy of this licensing report dated 11/13/25 that documents any Type A citation to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.

Per Title 22 Regulations and Health and Safety Codes, three(3) citations were issued. See LIC 9099D.


An exit interview was conducted, a copy of this report and appeal rights were read and provided to licensee Lisa Worthen.
Notice of Site Visit was provided and required to be posted for 30 days.

During records reviews on 8/28/25, LPA observed licensing forms from the files for C1 to have the date of birth (dob) manually altered on multiple licensing forms. Per contact with parent (dad) of C1, C1 has a dob of 07/07/23.

Licensee will ensure that when providing care and supervision to children enrolled, that there are no more than four children enrolled and provided care at any one time under the age of 24 months. During visit a parent was called and arrived to pick up an enrolled infant child while LPAs were present on site.
