On 12/23/24 Licensing Program Analyst (LPA) Ranita Richmond and Brittany Lovest arrived at the above named facility to conduct an unannounced case management deficiencies visit. LPA met with administrator, Kasa Adame.
LPA Richmond was conducting a file review and observed an incident (verbal disclosure). The director revealed while the disclosure did not occur at the facility it should have still been reported to CDSS. Director was reminded as a best practice to contact CDSS to find out if certain incidents are reportable. This is a type “B” violation and a potential risk.
Technical: While the facility generated an incident report on a child that sustained a bruise & scratches at the CCC. A best practice the CCC should always contact the parent to determine if the child requires assessment by a medical personnel.
At approximately 12:15pm LPA observed 13 children being cared for and supervised by 1 staff while awake on cots. Per administrator, " I'm on still on the premises." This is a type "B" violation and potential risk.
Exit interview conducted and copy of report and appeal rights were reviewed and provided to Kasa Adame.

A Notice of Site Visit was provided and must remain posted for 30 days.
