Per Center Director, there was no incident involving a staff yelling at day care children or staff not providing a sufficient quantity of food to children at the facility.

During LPA’s observation of care and supervision, staff were observed interacting with children during mealtime, hand washing, and indoor music activity. LPA did not observe any instances of children being spoken to in a loud tone of voice.

LPA reviewed menus posted which contain the minimum amounts of food components to be served at lunch. Snacks menus contain the minimum amount of food servings from at least two of the four components for Supplemental Food/Snack (Milk/Fluid, Vegetables/Fruits, Bread/Bread Alternates, Meat/Meat Alternates). Each snack includes at least one serving from each of two or more of the four major food groups. LPA made observation of meals being served and children eating. Lunch consisted of a chicken and peas casserole, 4 orange wedges, slice wheat bread, and milk.

Parents interviewed made no disclosures that supports the allegations above. Parent #6 stated that they are happy with care and facility always meets the needs of their family. Parent #8 stated that their children’s have attended this facility for over 12 years and have been happy with care. Parent #10 stated that their child enjoys attending and attached to their teachers.

Parents made no disclosures regarding facility not providing enough food. Parent #2 made disclosed that at time their child did not like the meals and would bring meals for their child. Parent #6 stated that meals were well balanced, and their children never came home hungry.

Based on the investigation conducted by the LPAs, it has been determined that although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is unsubstantiated.

The Notice of Site Visit must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00.

Exit interview conducted and report was reviewed with Tina Roberts, Center Director. ---Page 2 of 2
