Licensing Program Analyst (LPA) Lilia Hernandez conducted and unannounced complaint inspection to the above facility on 06/26/24. LPA arrived to the facility at 8:32AM and met with Tina Roberts, Director, and Tammy Jacquin, Lead Administrator, who guided LPA on a tour of the facility. There were 7 children with 1 staff upon arrival.

The purpose of the visit is to deliver findings for the above allegations.

Information provided by the reporting party indicates that staff are commingling the infants and the preschool children, staff did not report injuries to child's authorized representative, and staff did not properly supervise day care children.

During the investigation conducted by the LPA, interviews were conducted, records were reviewed, copies of the facility roster and other pertinent documents were obtained. ---Page 1 of 2

LPA interviewed staff who are responsible for the care and supervision of children. Staff interviewed made no disclosures to support the allegations. Staff #1 stated that both infants and preschool children are dropped off at the Dearborn location in their separate, assigned classrooms. When children are transported to this facility, infants and preschool children are brought separate to prevent any commingling between infants and preschool children. Staff disclosed that when an child is injured, injuries are assessed and first aid is administered. Parents are also notified by telephone or text and a written report is provided during pick up. Staff disclosed that children are always supervised and when needed, additional staff is provided.

LPA interviewed 6 parents who made no disclosures to support the allegations above. Parent #2 confirmed that both infant and preschool children have their designated drop off location and has never seen infant and preschool children cared for in the same classroom. Parent #3 confirmed that if their child has an injury while in care, they are notified by telephone and provided with a written report when their child is picked up. Parents made no disclosures regarding concerns with supervision. Parent #3 is happy with care and all the resources provide to their family.

During the course of the investigation, LPA did not observe children commingling. LPA observed the facility to be operating within its license capacity.

Based on the investigation conducted by the LPA it has been determined that although the allegations may have happened or are valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur, therefore the allegations are unsubstantiated.

A copy of this report, appeal rights, and Notice of Site Visit was provided.

The Notice of Site Visit must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00.
