25 | Licensing Program Analyst (LPA) Lilia Hernandez conducted an unannounced case management inspection due to an incident that occurred on 05/22/2024. LPA arrived at the facility at 8:55AM and met with Michelle Lomeli Patino, Designee, who guided LPA on a tour of the facility. LPA observed 13 staff present upon arrival and no children in care. Per Designee, facility is closed for care from June 13, 2024 to June 14, 2024 . Children return for summer program on June 17, 2024. LPA was later met by Meghan Eyer, Director.
The purpose of the visit was to follow-up on an incident that was reported to the department.
The incident that occurred on 05/22/2024, was reported to the Department on 05/23/2024, via telephone. The facility reported the Unusual Incident to the Department within the required 24 hours of occurrence.
Information reported to the Department indicated that Child #1 engaged in an activity that was not appropriate.
LPA conducted interviews and obtained documentation during today's visit.
Based on information LPA received from the interviews, there were no deficiencies cited during today’s inspection.
The Notice of Site Visit must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00.
