25 | On 6/18/24, Licensing Program Analyst (LPA) Ranita Richmond arrived at the above named Child Care Center to conduct an unannounced Case Management visit due to UIR received on 6/11/24.
On 06/11/2024, LPA Ranita Richmond received a call from the Site Supervisor regarding an Unusual Incident. The Site Supervisor reported that during lunch while eating a child began choking on his food. A teacher performed Heimlich and the food became dislodged. Parents were notified immediately. Child remained at school for the remainder of the day.
The site is a combo site with infant care. LPA toured facility preschool classrooms only inside and out and observed 27 children being cared for and supervised by 9 fingerprint cleared staff.
Based on the investigation which included interviews with relevant parties and observations by the LPA, there is no preponderance of evidence to prove there was a Lack of Supervision or any other health and safety violations occured.
Based on information obtained and interviews conducted, no further investigation is necessary.
