25 | Licensing Program Analysts (LPAs) Tatiana Bickham and Dawn Dowling conducted an unannounced annual required inspection at the above facility on 11/07/2024 at 8:15 AM. Upon arrival, LPA disclosed the purpose of the inspection and met with Ramzia Noorzay License, who guided LPAs on a tour of the facility. Licensee Mohammad Noorzay was not present during the inspection, per Ramzia Mohammad no longer assists with the day-care and wants to remove his name LPAs informed Ramzia to submit an updated LIC 279 to remove his name from the license.
Entrance Checklist – Family Childcare Homes LIC 126 was provided to the Licensee’s to help facilitate the inspection. Also present was Licensee’s spouse. There were (three) 3 day care children present during today’s inspection. Per Licensee, there are currently (fourteen) 14 children enrolled. Facility capacity is in compliance for a large Family Childcare Home. Hours of operation are Mon-Fri 6:00 AM - 6:00 PM, Licensee occasionally has children overnight and weekends.
LPA toured the home inside and outside. This is a one-story home which consists of 4 bedrooms, 3 bathrooms, kitchen, dining room, living room, laundry room, detached garage, kids’ playroom, front yard and backyard (fenced).
Per Licensee, areas off limits to children and parents include: All 4 bedrooms, 3 bathrooms, kitchen, laundry room, front yard, living room, dining room, detached garage, and front yard.
Per Licensee, areas that are accessible to children include: Kids playroom, bathroom #3, and backyard. The bathroom that children use is located at the end of the hallway on the right and was observed to be clean and free of hazards.
Food is provided by Licensee. Licensee was reminded if children bring food from home, it must be labeled with the child’s name and properly stored or refrigerated.
