On 02/03/2025 Licensing Program Analyst (LPA) Doris Whitmore conducted an unannounced Annual Required Inspection for the Infant License. LPA met Doris Phillips Facility operation hours are Monday through Friday from 7:00 AM to 6:00 PM.Children's sign in/out sheets were unable to be reviewed in the PRO- CARE App. Teaching staff were able to show LPA Where How many children were present for tody

Disinfectants, cleaning solutions, and other items that are dangerous to children, were inaccessible to children. Poisons are located in the locked storage closet. Furniture and equipment are in good condition, free of sharp, loose, or pointed parts. Facility has a functioning carbon monoxide detector that meets statutory requirements. The fire alarm system is directly connected to the local fire department. Doris Phillips states there are no weapons, firearms, or bodies of water on the premises. a fire extinguisher was observed. 3 A 10BC fire extinguisher indicates fully charged. All toilets and hand washing facilities are safe, sanitary and are operating properly. All floors are clean and safe. All kitchen areas/food preparation areas and food storage areas are kept clean and are free of litter, rubbish and rodents and/or any other vermin. All storage containers for solid waste, including moveable bins shall have tight-fitting covers that are kept on, and in good repair. Trash cans used to discard food have tight fitting lids. Drinking water is readily available both indoors and outdoors. The facility was observed to be free of flies, other insects and rodents.

Outdoor playground equipment is in safe condition, free of sharp, loose or pointed parts. The surface of the outdoor activity space is maintained in a safe condition and is free of hazards. All areas around or under high climbing equipment, swings, slides, and similar equipment are cushioned with material that absorbs a fall. There is adequate shade in the play yard.


Infant changing tables have a padded surface no less than one inch thick and are covered with washable vinyl or plastic and have raised sides at least three inches high. Toys were observed to be clean and safe. Infant napping equipment meets the requirements of California Code of Regulations (CCR) 101439.1 (a) - (f). Indoor activity space for this infant program is physically separate from the other components at this facility and meets the requirements of CCR 101438.3 (a) - (e).

and meets the requirements of CCR 101438.3 (a) - (e).
LPA reviewed a total of 5 files. Out of the 5 files there were two files that had the individual Sleeping Plan.LPA Whitmore discussed with Doris Phillips that the Personal Rights and Notification of Personal Rights forms, the top portions shall be given to the parent and the bottom portions shall be kept in the child's file. One file the the physicians report middle section was blank.LPA observed 2 staff files some files In one file the mandated training certificate expired 08/22/24. There was one person that was certified and current with First Aid and CPR. LPA observed in staff files a copy of the first aid and CPR Card. The course was provided by National CPR Foundation. LPA Whitmore discussed the importance of taking First Aid and COR in person and not online. Classes can be taken with American Red Cross or American Heart Association.
Menus are posted weekly on the parent board where it is visible by the child's authorized representative. Parent(s)/authorized representative(s) provide their own formula Or if they have a certain type of milk that they are on The facility provides one (1) afternoon snack.

Doris Phillips was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.


LPA discussed the safe sleep regulations with Director and discussed the Child Care Licensing Safe Sleep webpage at:https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed Director of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.. Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02- CCP. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. Doris Phillips was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California. LPA Whitmore was unable to review Lead Testing Information and will follow up at a later time and day.
