25 | Licensing Program Analyst (LPA) Tatiana Bickham conducted an unannounced annual required inspection at the above facility on 06/01/2026 at 12:50 PM. Upon arrival, LPA disclosed the purpose of the inspection and met with Orli Aziz Licensee, who guided LPA on a tour of the facility. Entrance Checklist – Family Child Care Homes LIC 126 was provided to the Licensee’s to help facilitate the inspection. Also present was Licensee’s assistant. There were (twelve) 12 day-care children (4 of whom were infants) present during today’s inspection. Per Licensee, there are currently (twelve) 12 children enrolled. Facility capacity is in compliance for a large Family Child Care Home. Hours of operation are Mon-Thurs 8:30 AM - 4:00PM and Fri 8:30 AM – 3:30 am.
LPA toured the home inside and outside. This is a one-story home which consists of 4 bedrooms, 3 ½ bathrooms, kitchen, den, living room, laundry room, converted garage, front yard (includes driveway) and backyard (fenced).
Per Licensee, areas off limits to children and parents include: 3 bedrooms (locked), bathroom #1 and #2, kitchen, den, laundry room, front yard, half of backyard (fence separating off limits area).
Per Licensee, areas that are accessible to children include: Bedroom #4, converted garage (daycare room), bathroom #2, bathroom #3, living room, portion of front yard (fence separating driveway) and a portion of the backyard(fenced). The bathroom that children use is located next to the backyard and in the daycare room, both were observed to be clean and free of hazards.
Food is provided by Licensee. Licensee was reminded if children bring food from home, it must be labeled with the child’s name and properly stored or refrigerated.
