Licensing Program Analyst (LPAs) Ashley Calderon conducted an unannounced annual random inspection and met with Kathy Graves. LPA disclosed the purpose of the inspection and was granted entry into the facility by personnel who resides at the home. Licensee was called to return to the facility for inspection.

Licensee gave consent for Kathy Graves to conduct tour with LPA. During today's inspection no children were present. The home is a single-story home. The home consists of 3 bedrooms, 1 bathroom, kitchen, dining room, living room, detached garage and backyard (fenced). Areas accessible to children are: Living room, dining room, 1 bedroom used for day care, bathroom, and backyard (fenced). Areas off limits to children and parents are: 2 bedrooms, detached garage and kitchen. Day care room has a door that connects to a off limit bedroom. When children in care room remains locked and inaccessible to children in care.

Individuals residing in the home have been discussed and noted, LPA observed 3 other individuals besides the Licensee whom resides at the home, individuals are fingerprinted and obtain record clearance. All areas identified on the facility sketch that are accessible for children to use were inspected for safety, comfort, and cleanliness. Licensee arrived at 1pm, LPA provided checklist and explained discussed the purpose of today's visit. Licensee gave a tour to the backyard, kitchen, day care room and bathroom.


LPA reviewed required posted documentation for Facility License, Publication (PUB) 394- Notification of Parent Rights and LIC 999A Facility Sketch or equivalent sketch. Facility records were reviewed for LIC 9040- Facility Roster, LIC 610- Facility Disaster Plan and Disaster drill log, last drill conducted on 7/15/22, LPA advised to complete every 6 months

Per Licensee, provides food/ snacks for children in care. During Licensee was reminded if children bring food from home it must be labeled with the child’s name and properly stored or refrigerated.

Per Licensee, smoke detector is hardwired to the Fire Dept. and carbon monoxide detectors was observed to be operable. Fire extinguisher indicated serviced and charged dated June 29,2023, inspection is done yearly. Disaster and Fire Drill conducted February 2024 and documentation reviewed. The home maintains telephone service via cell phone and landline. The home is observed to be clean and orderly. There are toys and other age appropriate material available for children. LPA observed that detergents, cleaning compounds and medication are inaccessible to children stored in the kitchen in high inaccessible cabinet. Licensee understands that all poisons must be locked, LPA observed poisons locked in garage. Per Licensee there are no firearms or weapons stored in the home and no pets living in this home.

Currently Licensee is not caring for any infants. Currently, children are using the backyard for outdoor play. The outdoor play area was observed to be fenced. LPA observed that the outdoor yard has toys and other materials for children to play with. LPA's did not observe any objects that could be hazardous to children in care. There are no pools or spas, or other bodies of water.

LPA reviewed 1 child's file. LPA reviewed records for child and all required documentation's were present.
LPA reviewed Licensee records, influenza is taken yearly per Licensee. Upon record review Mandated Reporter Training Certificate was found to be expired- 5/10/2022, deficiency noted on LIC 809-D.

Licensee [or facility representative] was reminded that all adults 18 and over living or working in the home, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated. LPA discussed the safe sleep regulations with licensee and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed licensee of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02-CCP. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. The following information regarding ADA was provided: US Department of Justice (USDOJ) toll-free ADA Information Line at (800) 514-0301 (voice)/ (800) 514-0383 (TTY) and link to publication: Commonly Asked Questions about Child Care Centers and the ADA, available at: https://www.ada.gov/resources/child-care-centers/.

Licensee was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

During the exit interview, the Licensee, confirmed that there are no Registered Sex Offenders living in the facility and LPA completed the RSO profile in FAS. Based on today’s visit, Per Title 22 regulation there were 1 deficiency given, see LIC 809-D.

A notice of site visit was given and must remain posted for 30 days.

Exit interview conducted and report was reviewed with the licensee Jacqueline Walker.
