On 03/10/2026 Licensing Program Analyst (LPA) Judy Laureano conducted an unannounced Annual/Random inspection at the above-mentioned facility. LPA was greeted by site director Christina Hector. LPA toured and inspected the facility in accordance with the facility sketch.

The following was observed during today’s inspection:
Classroom 44- no children were present during today’s inspection.
Classroom 45- 17 children present and 3 staff members. Classroom 45 is designated as the State Preschool Classroom.
Classroom 46- no children were present during today’s inspection
Classroom 47- 5 children and 3 adults present.
Classroom 48- 6 children and 3 adults present.

Facility operates on a school district campus. Days and hours of operation are Monday through Friday 7:50 a.m .to 10:50 a.m. and 12:00 p.m. to 3:00 p.m. Program offers an AM and PM session. Facility is licensed to serve 103 preschool children ages 2 years through entry to first grade. State Preschool operated; Tittle 5 Regulations apply to teacher child ratio.

Site director confirmed the following: Classroom 46, Classroom 47, Classroom 48 are used for the Special Education and program operates two sessions, morning session from 8 a.m. to 11:00 a.m. and an afternoon session from 12 to 3:00 p.m. Children enrolled are 3 to 4 years of age.

Facility will conclude services at the end of June 2026.

FACILITY EVALUATION REPORT California law requires a public report of each licensing visit/inspection. This report is a record for the facility and the licensing agency. This report is available for public review; therefore, care is taken not to disclose personal or confidential information. Inquiries concerning the location, maintenance, and contents of these reports may be directed to the Licensing Program Analyst or Regional Office whose address and telephone number are listed on the front of this form.

DEFICIENCIES A deficiency is an instance of noncompliance with licensing requirements, including applicable statutes, regulations, interim licensing standards, operating standards, and written directives. Applicants/ licensees must be notified in writing of all licensing deficiencies. Deficiencies are listed on the left side of this form, and the applicable licensing requirement upon which the deficiency is identified. There are two types of deficiencies:
  • Type A deficiencies are violations of licensing requirements that, if not corrected, have a direct and immediate risk to the health, safety, or personal rights of persons in care.
  • Type B deficiencies are violations of licensing requirements that, without correction, could become a risk to the health, safety, or personal rights of persons in care, a recordkeeping violation that could impact the care of said persons and/or protection of their resources, or a violation that could impact those services required to meet the needs of persons in care.

PLANS OF CORRECTION (POCs) The licensing agency is required to establish a reasonable length of time to correct a deficiency. In order to set the time, the licensing agency must take into consideration the seriousness of the violation, the number of persons in care involved, and the availability of equipment and personnel necessary to correct the violation. Applicants/licensees are requested to provide a specific plan for each violation on the right side of the form across from each deficiency. The more specific the plan, the less chance exists for any misunderstanding in setting time limits and reviewing corrections. The applicant/licensee who encounters problems beyond their control in completing the corrections within the specified time frame may request and may be granted an extension of the correction due date by the licensing agency.

CORRECTION NOTIFICATION The applicant/licensee is responsible for completing all corrections and promptly notifying the licensing agency of corrections. Applicants/licensees are advised to keep a dated copy of any correspondence sent to the licensing agency concerning corrections, or if corrections are telephoned to the licensing agency, the date, person contacted, and information given.

CIVIL PENALTIES The licensing agency is required by law to issue a Penalty Notice, when applicable, to all facilities holding a license issued by the licensing agency, or subject to licensure, except Certified Family Homes, Resource Families, and Foster Family Homes, or any governmental entity.

PENALTY NOTICE GIVEN The statement concerning civil penalties serves as a penalty notice on this Licensing Report and failure to correct cited licensing deficiencies will result in civil penalties. Applicants/ licensees are required to pay civil penalties when administrative appeals have been exhausted and in accordance with any payment arrangements made with the licensing agency.

APPEAL RIGHTS The applicant/licensee has a right without prejudice to discuss any disagreement in this report with the licensing agency concerning the proper application of licensing requirements. The applicant/ licensee may request a formal review by the licensing agency to amend or dismiss the notice of deficiency and/ or civil penalty. Requests for review shall be made in writing within 15 business days of receipt of a deficiency notification or civil penalty assessment. Licensing deficiencies may be appealed pursuant to the procedures in the LIC 9058 Applicant/Licensee Rights.

AGENCY REVIEW The licensing agency review of an appeal may be conducted based upon information provided in writing by the applicant/licensee. The applicant/licensee may request an office meeting to provide additional information. The applicant/licensee will be notified in writing of the results of the agency review within 60 business days of the date when all necessary information has been provided to the licensing agency.

EMAIL REQUIREMENT Adult Community Care Facilities, Residential Care Facilities for the Chronically Ill, and Residential Care Facilities for the Elderly are required to provide and maintain an active email address of record with the licensing agency.

Staff was observed operating within proper ratios. Classrooms were observed and inspected. Classrooms were observed to have fine and gross motor activities centers. Children’s cubbies were observed in the classroom. Furniture and equipment are in good condition, free of sharp, loose or pointed parts.

LPA observed allergy/medical list posted in the classroom. Medication, if needed, is stored in the classroom, in the top cabinet of the class; inaccessible to the children. If medication needs to be refrigerated, medication will be stored in the Health Specialist Office. All necessary paperwork that needs to be included with medication is stored in the classroom with medication as well as in the health office.



Teachers do the day to day cleaning in the classroom. Disinfectants, cleaning solutions and other hazardous items are made inaccessible in locked classroom cabinets. School district maintenance comes daily after school and mid-day to disinfect classrooms.

Playground equipment is in safe condition, free of sharp, loose or pointed parts. The surface of the outdoor activity space is maintained in a safe condition and is free of hazards. All toilets and hand washing facilities are in safe and sanitary operating condition. Floors in the facility are clean and safe.

Facility provides meals through the school district. Menu is posted in the classroom and parents receive it via Class Dojo online application and school district website. All food is protected against contamination and any contaminated food is discarded immediately. Preschool program coordinates with school cafeteria staff to ensure that all dietary restriction are being adhere too.

Drinking water is available both indoors and outdoors. Children bring in their own refillable water container and school will provide water bottles when children forget to bring in their water or run out of water.

Areas around high climbing equipment and slides have cushioning material to absorb falls. The facility is free of flies, insects and rodents. LPA observed working fire extinguishers and working carbon and smoke detector in each classroom. First Aid kits were observed in each classroom.

Capacity and limitations as specified on the license are being maintained.

At least one person trained in CPR and Pediatric First Aid is present when children are at the facility or at offsite activities. Director’s Pediatric CPR and Pediatric First Aid was taken on 8/7/2025. LPA observed 4 staff’s files and all had a valid certification and all the necessary Community Care Licensing forms. LPA reviewed Mandated Reporter Training and staff has current proof of completion. LPA reviewed a sample of children’s files and observed 6 files were complete.

The person who signs the child in/out of the facility shall use their full legal signature and record the time of day. Facility currently using Care Connect as an electronic sign in/out method and hard copy is available for LPA’s review.

Site supervisor was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.

Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02-CCP. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. Site supervisor was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.
