25 | Licensing Program Analyst (LPA) Roberto Luque Avila conducted an unannounced Case Management Plan of Correction visit to the above facility on 11/21/2024. LPA arrived at the facility at 2:15PM, identified self and met with Licensee Araceli Iraheta, who guided analyst on a tour of the facility. LPA observed 10 children 2 staff upon arrival. Per Licensee, operation hours are 6AM to 6PM. There are 14 children that are currently enrolled. The licensee is observed to be operating within the license capacity limitations
The purpose of this visit was to clear the deficiencies cited and technical violation given on 11/01/2024. On 11/01/2024 LPA cited the licensee with three Type B deficiencies and one Technical Violation. The three Type B deficiencies cited are as follow: 2 Fire Extinguishers with expired service tags, no record of 15 min infant sleep log, and facility sketch not matching the actual configuration of the home. The technical violation was for an empty above ground pool that was to be removed from the home.
During todays visit LPA observed 2 Fire Extinguishers with service tags dating 11/5/2024. LPA obtained an updated copy of their new facility sketch to include all the new off limits areas. LPA also obtained documentation of the infant 15 min sleep check. LPA did not observe an above ground pool in the home at this time and licensee stated that they recycled the pool.
Deficiencies were cleared on todays POC Visit.
A notice of site visit was given and must remain posted for 30 days.
Exit interview conducted, appeal rights and report were provided to the licensee Aracely Iraheta.
