Licensing Program Analyst (LPA) Doris Whitmore conducted an unannounced Required 3 year inspection. LPA met with Director, Kim Johnson. LPA inspected the inside and outside of the facility. School hours are 7:30a.m. to 5:30p.m. Monday through Friday. School offers both full and half day programs. Summer program is currently in session.A total of 55 children and 9 teachers were present during the inspection. LPA observed children during freeplay & outdoor activities.. Child Development Center utilizes 16 classrooms for learning. At the time of .LPA observed 3 classrooms in session. Additional classrooms were inspected but no children observed in the classrooms. LPA observed Fire Extinguishers and Carbon Monoxide

Furniture and equipment were inspected for age appropriateness and good repair in each classroom. Telephone service, heating, lighting and ventilation were evaluated. LPA observed individual cubbies for children's belongings in each classroom. The facility uses the health office as an isolation area when children are sick. The facility also have two other rooms for the isolation area. Age appropriate sinks and toilets were inspected for availability and good repair. LPA observed restrooms in good repair and a sufficient amount of toilets and sinks to accommodate campus capacity. Toilet paper, paper towels were observed inside each restroom. First Aid supplies are kept in individual classroom and health office. LPA observed fire extinguishers in each classroom and sign in/sign out sheets maintained. Fire and disaster drills are conducted monthly. LPA observed mats with sheets for napping in each classroom.

The center does provide breakfast and lunch and the parents provide afternoon snack. Each classroom has a water dispenser. The water dispenser is located in each classroom children bring their own water bottles. LPA observed water bottles labeled with children's names on them.Staff files were not observed. All staff files are located at the district office and will be reviewed at another time. LPA reviewed a total of 10 children files all documentation except one form for the physician report which was located in Aeries a system that is used to have several forms.

There were no bodies of water observed in the playground area. Facility has an upper and lower playground used for children. The outdoor playgrounds were inspected and observed to be free of hazards, loose and sharp parts. Adequate shade was observed for children in care. The playgrounds were observed to be properly gated all around. Equipment was inspected for safety, cushioning material, good repair and age appropriateness. LPA observed a gated patio for sitting, snack and crafts outside of classrooms.

The following was discussed with the licensee:
Mandatory Forms for the children’s files and provider’s files, Requirements for fire drills, earthquake drills and documentation for both. Role and responsibilities of being a mandated reporter were discussed. The licensee was advised how to access forms and Regulations for Family Child Care online at www.ccld.ca.gov . Licensee was made aware that it is their responsibility to know the regulations as well as anyone who assists in providing care. Licensee was advised on how to access quarterly reports, forms, and regulations for Child Care online at www.ccld.ca.gov. The licensee was advised that inaccessibility of hazards must be constantly reassessed depending on the children in care. Licensing must have the facility’s phone number at all times; if the phone number is changed, licensing must be notified. Regulation prohibits the smoking of tobacco on the premises.
There were no bodies of water observed in the playground area. Facility has an upper and lower playground used for children. AB 1207: Beginning on January 1, 2018, this law requires all licensed providers, applicants, directors and employees to complete training as specified on their mandated reporter duties and to renew their training every two years. Senate Bill 792: this bill, commencing September 1, 2016, prohibits a person from being employed or volunteering at a child care facility or family day care if he or she has not been immunized against influenza, pertussis and measles.

New Immunization Requirement: Law enacted by SB 277, beginning January 1, 2016, personal beliefs exemptions will no longer be an option for the vaccines that are currently required for entry into child care or school in California. Personal beliefs exemptions already on file will remain valid until the child reaches the next immunization checkpoint.

New Appeal Process: A licensee may file an appeal, in writing 15 business days from the date of receiving the penalty assessment.

The ADA requires that child care providers not discriminate against persons with disabilities on the basis of disability, that is, that they provide children and parents with disabilities with an equal opportunity to participate in the child care center's programs and services.



Requirements for fingerprint clearances and associations were discussed with the licensee. Licensee can be cited a civil penalty of $100 per day, up to $500.00 (5 days) for the 1st offense and up to $3000.00 for the 2nd offense within a 12 month period. The licensee was advised of the requirement to report unusual incidents and/or injuries to the parent/guardian and licensing within the time frame specified by the regulation. The "Notification of Parent's Rights" (PUB394) poster must be posted in an area accessible to parents. The information regarding new legislation with regards to exemptions and Parent’s Rights was also discussed.

The licensee was advised that the Notice of Site Visit must be posted at the entrance of the facility for a period of 30 days. If a serious violation is cited, a copy of the licensing report (LIC809 or LIC9099) must also be posted for 30 days. If these requirements are not met, civil penalties in the amount of $100 per violation will be assessed. Copies of the reports must also be provided to each parent when a serious deficiency, Type A, is cited (LIC9224).

CCAP participates in many community activities and special projects in order to disseminate information on the State’s licensing role, provide information to the public and parents on child care licensing, and provide many other helpful resources to the licensees and the public. Incidental Medical Services Discussed: Facilities that provide Incidental Medical Services (IMS) must identify those services in their facility’s Plan of Operation and submit an updated Plan of Operation to the Department. Incidental Medical Services Include: Blood-Glucose Monitoring for Diabetic Children, Administering Inhaled Medication, Administering EpiPen Jr. and EpiPen or other Epinephrine Auto-Injectors, Glucagon Administration, Gastrostomy Tube Care (G-tube care), Insulin Injections Administration, Anti-Seizure Administration, and Emptying an Ileostomy Bag.

This facility provides Incidental Medical Services – IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. The facility was not found to be in compliance per Title 22 regulations, Type A deficiencies will be cited today 08/10/2022
