On 4/15/2025, Licensing Program Analyst (LPA), Loyce Phillips, conducted a visit for the purpose of delivering the findings on the above allegation. LPA met with Susana Anton, Program Supervisor as the Director was unavailable today. LPA toured the facility and observed 34 children in care with 6 staff members.

LPA conducted a full investigation that included staff and parent interviews. Staff 1 (S1) disclosed she was responsible for leaving child 1 (C1) in a locked classroom for approximately 30 minutes. Staff 2 (S2) stated he unlocked the classroom for the parent and observed C1 sitting down at a table. S2 stated the child could have been in the classroom for about 45 minutes to 1 hour by himself. Parent arrived at the facility for pick-up and was unable to locate C1. Parent stated he found his child in classroom 11 with the door closed and locked with the lights off. Parent disclosed child was left in the classroom for 52 minutes by himself.

Based on the information obtained the allegation of staff left day care child locked in a classroom alone is deemed substantiated. A Substantiated finding means that the allegation is valid because the preponderance of evidence standard has been met.
Deficiency is cited in accordance with Title 22 of the California Code of Regulations and/or Health & Safety Codes.

LPA, Loyce Phillips informed Susana Anton, Program Supervisor that this report dated 4/15/2025 document(s) 1 Type A citation which shall be posted for 30 consecutive days as there are immediate risks to the health, safety, or personal rights of children in care.

Also, LPA, Loyce Phillips informed Susana Anton, Program Supervisor to provide a copy of this licensing report dated 4/15/2025 that documents any Type A citations to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.

The notice of Site Visit (LIC 9213) must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to maintain posting as required will results in a civil penalty of 100.00.

An exit interview was conducted, and a copy of this report and appeals rights was provided to Susana Anton, Program Supervisor.

Director will submit new transitioning
