Licensing Program Manager (LPM) Rita Ramos and Licensing Program Analyst (LPA) Lilia Hernandez conducted and unannounced complaint inspection to the above facility on 06/27/23. LPM and LPA arrived to the facility at 8:15AM and initally met with Vino Veerasingam, Administrator Assistant, who guided LPA on a tour of the facility. There were 14 children with 9 staff upon arrival. LPM and LPA were later met by Evan Levi, Licensee, Mia Levi, Licensee, and Debrah Heim, Director.

The purpose of the visit is to deliver findings for the above allegations.
During the investigation conducted by the Department interviews were conducted, records were reviewed, pictures were obtained, copies of rosters and other pertinent information and documents were also obtained. ---Page 1 of 2

Information provided by the reporting party indicates that Child #1 was not supervised while in care and Child #1 was also placed in crib with loose objects such as a blanket.

Director states that infants are supervised and when napping they are checked on every 15 minutes while in care.

Based on the investigation conducted by the Department it has been determined that although the allegations may have happened or are valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur, therefore the allegations are unsubstantiated.

The Notice of Site Visit must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00. Exit interview was conducted with Debrah Heim, Director, including, but not limited to Provider Rights, Appeal Procedures and Agencies Consultative Role. ---Page 2 of 2

Information provided by the reporting party indicates that Child #1 was wrapped in a blanket while in care.

Director states that per parents, parents want their children to be swaddled, however, per licensing swaddling is not allowed and staff were informed not to swaddle.
Staff #1 and Staff #2 disclosed both verbally and in written statements that Child #1 was swaddled while in care.

Based on the investigation conducted by the Department which includes interviews, observations, record reviews, and other pertinent information and documents, the preponderance of evidence standard has been met, therefore the above allegation is found to be SUBSTANTIATED. California Code of Regulations, Title 22, Division 12 Chapter 1 101429(a)(1) Responsibility for Providing Care and Supervision for Infants is being cited on the attached deficiencies
