32 | Per Director, care and supervision is continouslly addressed with staff and continues to ensure care and supervision regulations and guidelines are being followed.
LPA conducted interviews with Staff #1 through Staff #9. During the interviews, staff disclosed concerns with Staff #10 and submitted pertinent infoirmation regarding their concerns with Staff #10. However, staff could not confirm observing or witnessing Staff #10 violating the personal rights of children in care. Staff also reported incidences that may have occured in 2023 and 2024 and did not report the incidences to the Department within the required reporting time.
LPA also conducted interviews with Parent #1 through Parent #5. During the interviews there was no information provided to support the allegation or verify the information provided in the details of the allegation.
Per Director, there were instances where staff reported concerns with Staff #10. Director disclosed that in following up on the concerns, it was disclosed that their was conflict between staff members but was not able to find evidence of children's personal rights being violated.
Based on the investigation conducted by LPA, it has been determined that although the allegations may have happened or are valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation are unsubstantiated.
The Notice of Site Visit must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00.
A copy of this report, appeal rights, and Notice of Site Visit was provided.
