1/25/2023 Licensing Program Analysts (LPA) Judy Laureano conducted unannounced Annual Required Inspection at 1018 Broadway Ave., Santa Monica, CA 90404 LPA was greeted by Nancy Salazar, Site director. LPA toured the facility indoors and outdoors. LPA observed all classroom in the facility and observed 4 groups with 7 staff members and director/staff providing care and supervision. LPA observed the following classrooms:
Room #1 Art/Library Room
Room#2 Cozy Room.
Room #3 -Room #4- Room #5

Facility currently operates Monday through Thursday 8:30 am to 4:00 pm and Friday 8:30 am. to 2:30 p.m.

During today’s inspection, 48 children were present. The preschool serves 57 preschool children ages 2 years old up through 6 years old. LPA toured and inspected the facility in accordance with the facility sketch. Teacher/child ratios are maintained, and adequate supervision is being provided during this visit.

There is no swimming pool or other bodies of water on the premises. There are no firearms or ammunition allowed or stored on the premises.

Disinfectants and cleaning solutions are kept in each classroom, top cabinet making the content inaccessible to the children in care. Additional cleaning supplies and disinfectants are kept in the hallway locked closet and under the kitchen cabinet, locked and inaccessible to the children in care. Director confirmed facility has a designated cleaning crew that comes and cleans during the evening. Teachers are responsible for the daily cleaning in between classroom activities. Outside pest control vendor is contracted to come monthly to ensure that facility stays pest free. Facility is free of flies, insects and rodents. No poisons were observed during the inspection.

First Aid Kit observed in each classroom. Medication is stored in a locked storage box kept in the director’s office- making the content inaccessible to the children in care.
Furniture and equipment are in good condition, free of sharp, loose or pointed parts. Playground equipment is in safe condition, free of sharp, loose or pointed parts.

Facility currently has an outdoor waiver issued on 9/23/2021.T he surface of the outdoor activity space is maintained in a safe condition and is free of hazards. LPA observed shaded area in the outdoor space with age appropriate play equipment and toys. Areas around high climbing equipment have cushioning material to absorb falls. Teachers ensure playground and outdoor equipment is safe and free of hazard before children use the area.

LPA observed director removed one children’s tricycles that needs repair- a handlebar cover. Director agrees to make the item inaccessible until repairs are completed.

All toilets and handwashing facilities are in safe and sanitary operating condition. Floors in the facility are clean and safe. All kitchen, food preparation and storage areas are clean, free of litter/rubbish and free of rodents/vermin. Director was advised to add handwashing signage in all bathrooms.

Facility does not provide meals and/or snacks. All food is protected against contamination and any contaminated food is discarded immediately. Families are responsible for ensuring snacks and lunches are brought to facility in appropriate lunch boxes/bags (e.g. ice packs and/or thermos). Drinking water is available both indoors and outdoors, filter water is provided in each classroom and children are encouraged to bring their refillable water bottle.


Solid waste storage containers have tight-fitting covers and are in good repair. There is a working fire extinguisher, smoke detector, carbon monoxide detector and adequate heating and ventilation for safety and comfort.


Prior to working or volunteering in a licensed child care facility, all individuals subject to a criminal record review have received a criminal record clearance or exemption. Upon notification from the Department, the licensee will comply and act immediately to terminate the employment of, remove from the facility or bar from entering the facility for any person it is deemed necessary while the Department considers granting or denying an exemption. Capacity and limitations as specified on the license are being maintained.

LPA reviewed 8 staff files and observed 2 files were incomplete. Educational transcripts were not available for review. LPA issues a Technical Adivsiory. Director’s Mandated Reporter was completed on 8/7/2021 and CPR and 1st Aid completed with an expiration date of 9/2023. LPA reviewed children’s files and observed files were complete with contact information for authorized representative and or relatives or others who can assume responsibility for the child and medical assessment

LPA provided site director with a copy of LIC311A and LIC 125 to use as a guide to audit files and program.
The person who signs the child in/out of the facility shall use their full legal signature and record the time of day. All children are under supervision, including visual supervision, of a teacher at all times. Facility maintains a ratio of one teacher supervising no more than 12 children in care.

Facility representative, Nancy Salazar, was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

Incidental Medical Services (IMS) currently being provided. Licensee is aware that an IMS plan is required to be submitted to the licensing office if they provide any of these services. LPA and facility representative discussed the Community Care Licensing website www.ccld.ca.gov which will provide access to Provider Information Notices (PINs), Quarterly Updates, COVID-19 Information and Resources, Mandated Reporter Training, Safe Sleep in Child Care, Lead Poisoning Facts, Forms and Regulations.
A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. Exit interview conducted and report was reviewed with the licensee facility representative, Nancy Salazar.
