25 | On 1/23/25 Licensing Program Analyst (LPA) Ranita Richmond and Brittany Lovest arrived at the above named home to complete and unannounced Case Management Deficiencies visit. LPA was greeted by licensee Cecilia Penaloza. LPA toured the home inside and outside for health and safety.
During walk through, LPA observed disinfectant wipes, acetone, beauty products, and other items that could pose danger to children in care stored in the following areas accessible to children in care: the middle medicine cabinet (acetone and beauty products), and on the counter inside the classroom (clorox disinfectant wipes). Citation cited. Type B. LPA witnessed infant child placed on stomach to sleep in play pen. LPA reviewed infant sleep log. Infant sleep log agrees that infant child was placed on stomach to sleep. Infant sleep log has date, time, staff initials, and position of infant sleeping. Citation issued. Type B. repeat Violation. Civil penalties assessed.
LPA reviewed staff file for L. Esquivel. Staff file does not contain TB test dated current from hire date. Citation cited. Type B.

An exit interview was conducted, a copy of this report, and appeal rights were discussed and provided to licensee Cecilia Penaloza.
